Supportive housing: clare housing provides permanent affordable housing and supportive services to 77 low-income residents who have experienced homelessness, long-term homelessness or are at risk of homelessness at two supportive housing sites: clare apartments and clare midtown. These sites also provide assisted living opportunities for residents who need additional supportive services, such as, nursing care, medication administration, hands on assistance with activities of daily living, and building independent living skills. The residents of clare housing represent the diversity of people most impacted by hiv/aids in our community: 38% african american, 12% latino, 6% american indian, and 44% caucasian; 17% women and 83% men. The average age of residents in the community care homes is 49 and in the supportive housing apartments is 40. More than 50% of clare housing residents have a mental health diagnosis, 70% have a history of chemical dependency, and 50% have been homeless at some point in their lives. In 2013, clare housing began the planning process for two new affordable 36 unit apartment buildings for people living with hiv/aids with an anticipated opening in 2016.

Activity 3

Project cornerstone - clare housing provided scattered-site supportive housing for twenty individuals/families throughout the twin cities metro area. Housing is provided for households that meet the state of minnesota's definition of long term homelessness, with a target population of individuals and families that are living with hiv/aids.
