“During 2018, three new programs were offered at Guild. The Coordinated Entry Navigation Service (CENS) as a pilot program, staffed with one FTE, provides navigation services to individuals experiencing long-term homelessness and rise to the top of the "priority list" managed by the Continuum of Care (County or County collaborative). This partnership is with Hearth Connection and the COC's in the Suburban Metro Area, including Hennepin and Ramsey Counties. The HCSB Innovations Grant through DHS has also allowed the funding of an IPS Employment program which began in July 2018. The program offers support to individuals to improve employment outcomes by securing competitive employment that offers minimum wage or above. The target population is individuals ages 18 to 25, and adults with serious or serious and persistent mental illness in Dakota and Ramsey County. The program assists individuals in securing and retaining employment that matches their skills and abilities. The program also works with the individual on career advancement. Priority for services is given to individuals with SSI/SSDI, students currently or previously served at the federal education setting level 3 or 4, and/or individuals who are CADI waiver eligible. In January 2018, Guild was asked to assume a PATH grant which funded a .5 FTE to engage potential PATH clients through street and referral outreach. The goal is to determine eligibility for the PATH program and enroll individuals into the program if eligible. The case manager also assists with connection to services, financial or direct assistance, assists with applications for SSI/SSDI using SOAR method assistance, disbursed outreach items (i.e. hygiene supplies), provided resource navigation, case management, and housing support in finding housing resources. The case manager coordinated closely with Dakota County staff and Coordinated Entry regarding referrals and working with participants to get assessment (VI-SDAT) needed for Coordinated Entry and placed on the priority wait list.”

Due to a strategic financial shift by one of the contracted plans, which resulted in a 60% reduction of clients, the team was required to reduce by 4 FTE's between June and September 2018. The agency was able to anticipate the changes early in order to prepare for a successful and seamless transition of clients back to internal health plan Care Coordinators. In addition, the agency retained two of the four staff impacted through open positions in other program areas.”

Guild is annually required to file a Return of Organization Exempt from Income Tax (Form 990) with the IRS. In addition, Guild is subject to income tax on net income that is derived from business activities that are unrelated to its exempt purposes. Guild has determined it is not subject to unrelated business income tax and has not filed an Exempt Organization Business Income Tax Return (Form 990-T) with the IRS. Guild believes that it has appropriate support for any tax positions taken affecting its annual filing requirements, and as such, does not have any uncertain tax positions that are material to the financial statements. Guild would recognize future accrued interest and penalties related to unrecognized tax benefits and liabilities in income tax expense if such interest and penalties are incurred.”

The average length of stay was seven days. Eighty-eight percent of admissions resulted in the individuals stabilizing their situations without hospitalization for psychiatric care. Sixty-five individuals were served in the nine-bed intensive residential treatment services program, with an average length of stay of 58 days. |

In 2018, 425 individuals were served in this model. Delancey Apartments represents a collaboration between Guild Incorporated and Project for Pride in Living, providing permanent, supportive housing for people experiencing chronic homelessness. 13 individuals benefitted from this housing in 2018.Guild's Hospital to Home Program, a partnership with Regions Hospital, Hearth Connection, and the MN DHS, is demonstrating a reduction in avoidable high-cost health services while increasing stability in health and housing for low-resource homeless individuals with complex healthcare needs. In 2018, Guild's Hospital to Home served 43 individuals and has demonstrated a consistent decrease after enrollment in emergency department usage. Guild partners with Dakota County to help house people living in shelters who have significant barriers to housing. Sixty-one formerly homeless individuals are now housed and have access to on-going housing supports to aid in maintaining stable housing. During 2018, Guild issued 144 housing subsidies and vouchers. |

In 2018, 73% of individuals served were placed in competitive employment or maintained employment from the prior year. Nationally, only 10-15% of people living with a serious mental illness are working. Based on placement rate, Guild's program is in the top 25% of similar programs in the nation that use the evidence-based IPS model. Guild's community support program helps individuals develop, restore, and enhance their psychiatric stability, social competencies, personal and emotional adjustment, and community living skills. One hundred and fifty-seven individuals made 5,918 visits to the center, participating in both center-based and community-based activities.Volunteer Services: More than 216 community members help Guild Incorporated meet its mission and purpose through volunteer service. |
