Mission

The mission of phillips eye institute foundation (peif) is to further the commitment of phillips eye institute (pei) to eye care excellence, education, research and community involvement while honoring the legacy of mount sinai hospital.

See schedule o.what we do:1. Provide free access to superb medical care through transportation services for our patients.2. Conduct free annual vision screening for nearly 30,000 public school children and fund and coordinate follow-up treatment for children who are uninsured or underinsured.3. Purchase cutting-edge technology, which allows physicians to better diagnose and treat our patients.4. Fund education on glaucoma and other eye diseases and issues.who we do it for:the work of phillips eye institute, the upper midwest's only eye hospital, is so vital to our region. We integrate the latest diagnostic treatment and surgical technologies with a staff dedicated solely to ophthalmic care, the expertise of affiliated ophthalmologists, referring physicians and other eye care professionals. This unique collaboration ensures that patients receive the latest in advanced eye care-all within a single hospital dedicated to preserving, protecting and restoring vision.

Allina health system is the sole corporate member organization of phillips eye institute foundation. Phillips eye institute foundation is the supporting organization and allina health system is the supported organization. See schedule a and schedule r for further details. Allina health system provides phillips eye institute foundation with administrative and staff services at no cost. The value of the services provided by allina health system to phillips eye institute foundation during 2013 were estimated at $188,600 and have been recorded as contribution revenue from a related organization in the statement of revenue and as administrative and fundraising expenses in the statement of functional expenses. Phillips eye institute foundation has no employees and does not pay compensation. Compensation disclosed as paid by a related organization is determined solely by the related organization.

Form 990, Part VI, Section A, Line 7A

As the sole corporate member organization, allina health system, holds certain reserved powers over phillips eye institute foundation. The reserved powers are fully described in phillips eye institute foundation's governing documents and include such powers as election and approval of directors, final approval of amendments to articles of incorporation or bylaws, approval of strategic plans and capital and operating budgets, approval of plans of merger or consolidation, approval of incurrence of debt or expenditures in excess of certain amounts, and various other reserved powers as described therein.

Form 990, Part VI, Section B, Line 11

Phillips eye institute foundation's form 990 was prepared by the tax services function of allina health system, phillips eye institute foundation's sole corporate member and supported organization. The form 990 filing was subjected to a rigorous review process by allina's tax manager and tax director. Allina's vice president of finance & treasury also performed an executive review of the form 990. After the management review process described above was completed, the final form 990, as ultimately filed with the internal revenue service ["irs"], was provided to each voting member of phillips eye institute foundation's board of directors. A phillips eye institute foundation board of directors meeting was held on september 18,2014 to review and discuss the form 990 filing. Phillips eye institute foundation's board of directors voted on and approved a resolution approving the form 990 and the minnesota charitable organization annual report to be filed with the minnesota attorney general. The board of directors resolution also directed officers to file the form 990 with the irs and the charitable annual report with the charities division of the office of the minnesota attorney general. The above stated review and approval process occurred prior to filing phillips eye institute foundation's form 990 with the irs and minnesota charitable organization annual report with the minnesota attorney general.

Form 990, Part VI, Section B, Line 12C

The organization has several methods of monitoring and enforcing compliance with its conflict of interest policy. First, the organization regularly distributes conflict of interest disclosure questionnaires to its officers, directors, trustees, and key employees. These individuals are required to disclose annually any interest that could give rise to conflicts, including any family or business relationship. Second, the general counsel's office annually delivers a report to allina's board of directors which includes, among other things, the results of the conflict of interest questionnaire, an analysis of potential conflicts, and guidance for satisfactorily resolving conflicts. Third, the organization undertakes mandatory compliance training of all its employees which includes training on conflicts of interest. Fourth, all employees receive, and are expected to conduct themselves in accordance with allina's code of conduct. The code of conduct contains educational materials and guidance to resolve potential conflicts of interest. Fifth, allina maintains a corporate integrity hotline, a confidential 24 hour external resource to help answer questions related to ethical business conduct. All calls to the integrity line are kept confidential.

Phillips eye institute foundation has no employees and does not pay compensation. Any compensation disclosed as paid by a related organization is determined solely by the related organization. Therefore, part vi, section b, line 15a and line 15b are not applicable to philips eye institute foundation.

The following is a copy of footnote 1 to the audited financial statements: summary of significant accounting policies - income tax "the foundation has received a determination letter from the internal revenue service indicating that it is exempt from federal income tax under section 501(a) of the internal revenue code (irc) as an organization described in section 501(c)(3) and 509(a)(3) and that charitable contributions by donors are deductible under section 170 of the irc. The foundation has adopted guidance in the income tax standard regarding the recognition of uncertain tax positions. The guidance prescribes recognition threshold principles for the financial statement recognition of tax positions taken or expected to be taken on a tax filing that are not certain to be realized. The implementation of this guidance had no impact on the foundation's financial statements. The foundation's tax filings are subject to review and examination by federal and state authorities. The foundation is not aware of any activities that would jeopardize its tax exempt status. The foundation is not aware of any activities that are subject to tax on unrelated business income, excise or other taxes. The filings for the years ending 2010 to 2013 are open to examination by federal and state authorities."

APPROXIMATELY 80% OF CHILDHOOD LEARNING IS VISUALLY BASED, MAKING HEALTHY VISION A KEY TO ACADEMIC SUCCESS. GIVEN THAT 20-25% OF CHILDREN HAVE VISION PROBLEMS THAT CAN AFFECT THEIR PERFORMANCE WITHIN AND OUTSIDE THE CLASSROOM, EARLY DETECTION AND TREATMENT OF VISION PROBLEMS IS CRITICAL TO A CHILD'S DEVELOPMENT.SINCE THE PROGRAM'S INCEPTION IN 2008, WE CONDUCTED OVER 73,000 SCREENINGS FOR POTENTIAL VISION PROBLEMS.AS CRITICAL AS VISION SCREENING IS, IT ISN'T ENOUGH - CHILDREN NEED TIMELY FOLLOW-UP CARE BASED ON THEIR SCREENING RESULTS. CONSEQUENTLY, EYE PROGRAM STAFF CONTACT THE PARENT/GUARDIAN OF EVERY CHILD WHO DID NOT PASS THE SCREENING TO ENSURE THEY ARE ABLE TO ACCESS NECESSARY VISION SERVICES. FOLLOW-UP COMMUNICATION IS CONDUCTED WITH OVER 8,000 FAMILIES, NURSES AND PROVIDERS ANNUALLY.MANY PARENTS/GUARDIANS FACE NUMEROUS CHALLENGES PURSUING VISION CARE FOR THEIR CHILDREN. FOR THOSE FAMILIES THAT REQUIRE FINANCIAL ASSISTANCE, TRANSPORTATION OR INTERPRETER SERVICES, ALL NECESSARY VISION-RELATED SERVICES ARE COMPLETELY FUNDED AND COORDINATED THROUGH THE EYE PROGRAM ON THE CHILD'S BEHALF. PARTICIPATING CLINICS PROVIDE NECESSARY EXAMS, TREATMENTS, GLASSES, AND FOLLOW-UP CARE AND ARE REIMBURSED THROUGH THE E.Y.E PROGRAM. ADDITIONALLY, THE PROGRAM PROVIDES PHARMACY, DIAGNOSTIC AND SURGICAL SERVICES TO CHILDREN IN NEED. THE E.Y.E PROGRAM HAS FUNDED AND COORDINATED CARE FOR OVER 1,400 PROGRAM BENEFICIARIES. PHILLIPS EYE INSTITUTE FOUNDATION IS THE FISCAL AGENT FOR THE E.Y.E PROGRAM, HAVING DEVELOPED DESIGNATED FUNDING TO PROVIDE COMMUNITY OUTREACH. ADDITIONAL SUPPORT INCLUDES THE PHILLIPS EYE INSTITUTE MEDICAL STAFF, EMPLOYEE GIVING CAMPAIGNS, PRIVATE AND CORPORATE DONORS, FOUNDATION GRANTS AND AN ANNUAL FUNDRAISER.IN 2013 OGI KIDS EYEWEAR, A LOCALLY BASED COMPANY THAT DISTRIBUTES FUN AND FUNCTIONAL FRAMES, AGREED TO CONTINUE THEIR DONATION WITH 430 FRAMES IN SUPPORT OF THE EYE PROGRAM. WALMAN OPTICAL,A LOCALLY OWNED LENS COMPANY AGREED TO SUPPLY THE NECESSARY LENSES FOR EACH CHILD'S GLASSES.
