HAVEN HOUSING

Form 990, Page 2, Part III, Line 4A

Ascension place (ap) provides private bedrooms, shared bathrooms, and 24- hour staff support for up to 32 women. Residents receive daily meals, snacks, personal care items, clothing, internet service, laundry facilities, and access to our library. Case managers provide individual and group support as women stabilize, heal, and set and work toward goals. Targeted referrals connect women with the resources in the community to assist them on their path to stability. Supportive programming for women at ap includes: twice-weekly mental health and chemical health groups, financial literacy classes, health and wellness activities and workshops, opportunities for "sober fun," community-building activities, and more. In 2018, ascension place (ap) provided transitional housing to 64 women.

Form 990, Page 2, Part III, Line 4B

St. Anne's place (sap) emergency shelter provides private family bedrooms and shared bathrooms, as well as personal care items, clothing and basic necessities, for up to 16 families. Our secured building is staffed 24 hours a day for the safety and security of our residents. Upon arrival, families meet with the sap team to help determine their immediate needs and each mother's and child's barriers. Staff collaborates informally and at our weekly resident review meeting to provide coordinated services for each member of every family. The family services coordinator assesses family needs and coordinates resources and services for families, including housing and employment searching, life skills, chemical addictions, legal issues, mental health support and overall family health and wellness. Our children's coordinator facilitates children's intakes, assessments and case management, coordinates communication between families and schools/pica (childcare), helps ensure that children at the shelter are equipped for school with school supplies, uniforms, etc., and leads indoor, outdoor and off-site children's activities. Supportive programming and activities at sap include: parent support group, children's mental health programming, wellness activities, child and family field trips, tutoring for school-age children, on-site music therapy, holiday and birthday celebrations, and more. In 2018, st. Anne's place provided housing and support to 118 mothers and 209 children.

Next step housing (nsh) provides permanent supportive housing to women and women-led families - through both site-based and scattered site programs. First, our haven housing-owned apartment building in north minneapolis provides secure, site-based housing for 11 single women. Second, we support women and women-led families in scattered site apartments throughout hennepin county. Case managers meet with clients in their homes to provide individualized support, resources and referrals, and to assist women and families with their goal plan. Nsh staff also provides assistance with landlord relationships and lease maintenance, education on independent living skills (cooking, cleaning, minor repairs, etc.), health and wellness coaching, assistance with education and employment, and more. Supportive programming for nsh clients includes: therapeutic arts activities, group outings, community-building activities, holiday and birthday celebrations, and more. In 2018, next step housing provided permanent supportive housing to 35 households (26 single women and 9 families with 11 children).

STAFF COLLABORATES INFORMALLY AND AT OUR WEEKLY RESIDENT REVIEW MEETING TO PROVIDE COORDINATED SERVICES FOR EACH MEMBER OF EVERY FAMILY. THE FAMILY SERVICES COORDINATOR ASSESSES FAMILY NEEDS AND COORDINATES RESOURCES AND SERVICES FOR FAMILIES, INCLUDING HOUSING AND EMPLOYMENT SEARCHING, LIFE SKILLS, CHEMICAL ADDICTIONS, LEGAL ISSUES, MENTAL HEALTH SUPPORT AND OVERALL FAMILY HEALTH AND WELLNESS. OUR CHILDREN'S COORDINATOR FACILITATES CHILDREN'S INTAKES, ASSESSMENTS AND CASE MANAGEMENT, COORDINATES COMMUNICATION BETWEEN FAMILIES AND SCHOOLS/PICA (CHILDCARE), HELPS ENSURE THAT CHILDREN AT THE SHELTER ARE EQUIPPED FOR SCHOOL WITH SCHOOL SUPPLIES, UNIFORMS, ETC., AND LEADS INDOOR, OUTDOOR AND OFF-SITE CHILDREN'S ACTIVITIES. SECOND, WE SUPPORT WOMEN AND WOMEN-LED FAMILIES IN SCATTERED SITE APARTMENTS THROUGHOUT HENNEPIN COUNTY.
