Governance Explanations

Form 990, Part VI, Section A, Line 1A

The finance committee has the authority to review and approve the fiscal procedures, fundraising plans, annual budgets, and annual tax returns. The finance committee consists of the treasurer as the chair and three other board members (more or less members as may be needed). The finance committee has no other authority on behalf of the full board.

Form 990, Part VI, Section B, Line 11B

Draft copies will be presented to governance at both a finance committee meeting and a board meeting for review and approval prior to filing.

Form 990, Part VI, Section B, Line 12C

Conflict of interest for board members is completed annually and disclosure is part of the monitoring process. Key employees and officers update code of conduct annually. It is the responsibility of the individual board member who perceives any potential conflict of interest to bring such conflict to the attention of the board of directors. The board will review the matter and determine whether the relationship is an acceptable one. The action of the board of directors should clearly be reflected in the minutes of the committee and/or the board meeting in which the situation was reviewed. Individual board members will be expected to abstain from votes on board matters in which board action would have financial impact on them or the firm they represent. Individual board members will also be expected to adhere to the organization's code of conduct and any professional standards applicable to the organization which they represent, when those restrictions would be more stringent than the requirements outlined herein. Key employees and officers review the organization's code of conduct annually. If a proposed transaction or situation raises any questions or doubts with respect to a potential conflict of interest, employees are required to discuss the facts with their manager or someone in the human resources department before entering into the relationship or situation in question.

Form 990, Part VI, Section B, Line 15

The compensation for the president is reviewed by select board members assigned to the task to gather comparable data from form 990 and salary survey information for our geographic location and complexity of operations. Then, the board will document the process contemporaneously, including the terms of the transaction, date of approval, voting members present, and any conflict of interest. The documentation will include the basis for determining the compensation, including the comparability data obtained and relied upon. The president conducts annual performance evaluations for other officers and employees. The president incorporates form 990 data and salary survey information for our geographic location and complexity of operations and documents adjustments in human resources files. These processes were last performed in 2024.

ABOUT 10% OF THE PEOPLE WE SUPPORT PARTICIPATE IN PROGRAMMING ACROSS MULTIPLE SERVICE DOMAINS.RISE SUPPORTS PEOPLE WITH DISABILITIES, BROADLY DEFINED. APPROXIMATELY 70% OF THOSE WE SERVE HAVE AN INTELLECTUAL OR DEVELOPMENTAL DISABILITY OR A BRAIN INJURY AS THEIR PRIMARY DISABILITY. TWENTY-FIVE PERCENT HAVE A PSYCHIATRIC DISABILITY, AND 5% ARE DEAF, DEAFBLIND, OR HARD-OF-HEARING. RISE ALSO SUPPORTS DISABLED POPULATIONS FACING COMPLEX BARRIERS TO STABILITY, INCLUDING INDIVIDUALS RE-ENTERING THE COMMUNITY AFTER INCARCERATION OR INSTITUTIONALIZATION, THOSE WITH CO-OCCURRING SUBSTANCE USE CHALLENGES, AND PEOPLE WITH CRIMINAL JUSTICE HISTORIES.EMPLOYMENT SERVICES:IN 2024, MORE THAN 1,400 PEOPLE PARTICIPATED IN EMPLOYMENT SERVICES ACROSS EIGHT DIFFERENT PROGRAMS. ABOUT 45% OF PARTICIPANTS CAME TO RISE THROUGH A MEDICAID WAIVER, WHILE 55% WERE REFERRED THROUGH OTHER FUNDING MECHANISMS. OF THESE 1,400 INDIVIDUALS, 40% WERE EMPLOYED IN A COMPETITIVE, INTEGRATED JOB DURING THE YEAR. SIXTEEN PERCENT STARTED A NEW JOB (N=217), AND 18% REACHED THE 90-DAY RETENTION MILESTONE (N=258). THE AVERAGE HOURLY WAGE FOR THOSE EMPLOYED WAS $16.83, WITH AN AVERAGE OF 23.8 HOURS WORKED PER WEEK. ADDITIONALLY, 28% OF PARTICIPANTS WORKED ON SUPPORTED EMPLOYMENT TEAMS (SET), EARNING AN AVERAGE HOURLY WAGE OF $10.99 (100% AT OR ABOVE MINIMUM WAGE) AND AVERAGING 28 HOURS PER WEEK.LIFE ENRICHMENT:IN 2024, 731 INDIVIDUALS PARTICIPATED IN LIFE ENRICHMENT ACTIVITIES AT A RISE PROGRAM, WITH 18% ENGAGING IN REMOTE SERVICES.HOUSING AND INDEPENDENT LIVING SUPPORTS:RISE SUPPORTED 150 PEOPLE EXPERIENCING HOMELESSNESS OR AT IMMINENT RISK OF HOMELESSNESS IN SECURING PERMANENT HOUSING OF THEIR CHOICE IN CENTRAL MINNESOTA. ANOTHER 100 INDIVIDUALS RECEIVED MENTAL HEALTH SUPPORT THROUGH ADULT REHABILITATIVE MENTAL HEALTH SERVICES (ARMHS) IN CENTRAL MINNESOTA AND ANOKA COUNTY.SERVICE AREA:RISE'S SERVICE AREA INCLUDES THE ENTIRE STATE OF MINNESOTA AND SELECT COUNTIES IN WESTERN WISCONSIN. SERVICES ARE CONCENTRATED IN THE TWIN CITIES METRO (73% OF PERSONS SERVED) AND CENTRAL MINNESOTA (18%), WITH THE REMAINDER SPREAD ACROSS OTHER MINNESOTA COUNTIES AND AREAS SURROUNDING NEW RICHMOND, WISCONSIN. WITHIN THE TWIN CITIES METRO, SERVICES ARE PRIMARILY LOCATED IN THE NORTHERN AND EASTERN QUADRANTS, WITH 26% OF PERSONS SERVED RESIDING IN HENNEPIN COUNTY, 25% IN ANOKA COUNTY, 13% IN WASHINGTON COUNTY, AND 9% IN RAMSEY COUNTY.DEMOGRAPHICS:RISE'S PARTICIPANTS REFLECT THE DIVERSITY OF THE COMMUNITIES THEY LIVE IN. TWENTY-FIVE PERCENT ARE UNDER 30 YEARS OLD, WHILE 31% ARE OVER 50, CONSIDERED OLDER ADULTS WITHIN THIS POPULATION. SEVENTY-FOUR PERCENT OF THOSE SERVED ARE WHITE, AND 36% ARE PEOPLE OF COLOR. THE LARGEST NONWHITE GROUP IS BLACK OR AFRICAN AMERICAN, COMPRISING 13% OF ALL PERSONS SERVED.ORGANIZATIONAL CHANGES IN 2024:2024 BROUGHT SEVERAL SIGNIFICANT AND ANTICIPATED CHANGES TO RISE. AFTER A MULTIYEAR PROCESS, RISE ENDED ITS SPECIAL MINIMUM WAGE PROGRAM EARLY IN THE YEAR.
