The three officers of the board first review the form 990 and then it is sent to all members of the board for their review prior to filing the return with the internal revenue service.

The executive director reviews all the disclosure of interest forms completed annually by our agency's employees for any conflicts of interest. If there are any potential conflicts among employees, the executive director determines if a conflict exists, and if so, what action will be taken to deal with the conflict of interest. The board president reviews the disclosure of interest forms completed annually by our board members. The executive director consults with the board president regarding any potential conflict of interest disclosed by a board member. The board president, in consultation with the other board officers, then determines if any action needs to be taken regarding the conflict of interest. Any person who has a conflict of interest shall not participate in or be permitted to hear the board's discussion of the matter and may not vote and shall not be present in the meeting room when the vote is taken.

For the executive director, the board of directors sets the compensation. Periodically (usually every 2 years) the board reviews comparability data re: executive director salaries of other similar organizations. For the other agency key employees, the executive director sets the compensation. The executive director generally obtains and reviews comparability data for various positions in other similar organizations every two years. The comparability study for all positions was last conducted in 2014.

DURING FISCAL YEAR 2015 FCIOP SERVED 584 INDIVIDUALS WITH 1,037 SERVICE REQUESTS. THE TOP PRESENTING ISSUES WERE: SERVICE, MEDICATION OR EQUIPMENT REDUCTIONS OR DENIALS OF NEW REQUESTS; SERVICE REDUCTION OR TERMINATION OF EXISTING SERVICES; ENROLLMENT, ELIGIBILITY OR DISENROLLMENT PROBLEMS; QUALITY ISSUES WITH PROVIDERS; UNWANTED RELOCATIONS; AND REQUESTED RELOCATIONS NOT ACTED UPON. INFORMAL RESOLUTION IS THE PRIMARY TACTIC BECAUSE IT EASES THE STRAIN ON MEMBERS AND THEIR FAMILIES, SAVES TIME AND MONEY, AND ALLOWS MEMBERS AND FAMILIES TO MAINTAIN A POSITIVE RELATIONSHIP WITH THEIR CARE TEAMS.
