A copy of form 990 is provided to the governing body before it is filed. The executive director and the chair review the entire form 990, provide changes as needed and suggest approval to the board.

Form 990, Page 6, Part VI, Line 12C

Conflicts of interest are reviewed routinely with semi-annual re-signing of the policy. Hcc staff must act at all times in the best interests of the organization and not for personal or third-party gain or financial enrichment. When encountering potential conflicts of interest, staff shall identify the potential conflict and, as required, remove themselves from all voting on the matter. Specifically, staff shall: - avoid placing (and avoid the appearance of placing) one's own self-interest or any third-party interest above that of hcc; while the receipt of incidental personal or third-party benefit may necessarily flow from certain hcc activities, such benefit must be merely incidental to the primary benefit to hcc and its purposes; - not abuse their position by improperly using services, equipment, materials, resources, or property for their personal or third- party gain or pleasure, and shall not represent to third parties that their authority extends any further than that which it actually extends; - not engage in any outside business, professional or other activities that would directly or indirectly materially adversely affect hcc; - not engage in or facilitate any discriminatory or harassing behavior directed toward hcc staff, members, officers, directors, meeting attendees, exhibitors, advertisers, sponsors, suppliers, contractors, or others in the context of activities relating to hcc; - not solicit or accept gifts, gratuities, free trips, honoraria, personal property, or any other item of value from any person or entity as a direct or indirect inducement to provide special treatment with respect to matters pertaining to hcc without fully disclosing such items; - provide goods or services to hcc as a paid vendor to hcc only after full disclosure to, and advance approval by, the board, and pursuant to any related procedures adopted by the board; - not persuade or attempt to persuade any employee of hcc to leave the employ of hcc or to become employed by any person or entity other than hcc; - not persuade or attempt to persuade any member, exhibitor, advertiser, sponsor, subscriber, supplier, contractor, or any other person or entity with an actual or potential relationship to or with hcc to terminate, curtail or not enter into its relationship to or with hcc, or to in any way reduce the monetary or other benefits to hcc of such relationship.

Health care clinic (hcc) empowers communities through specialized and inclusive reproductive services. Hcc envisions a world in which all people have access to just and dignified reproductive health care. Our commitment to our patients is to provide individualized healthcare based on the patient's individual needs. We pride ourselves on providing exceptional reproductive health services to the community. Our staff works closely with each patient to achieve and maintain health goals.in 2024, health care clinic (hcc) provided low or no cost reproductive health and family planning services to a total of 198 unduplicated patients.for more than 40 years hcc has provided the most up-to-date, evidence-based care. Hcc follows the wisconsin department of health services women's health-family planning/reproductive health program guidelines (2022) and the wisconsin family planning and reproductive health association's reproductive health best practice guidelines (2021). Our medical team also works under the united states medical eligibility criteria for contraceptive use (us mec, 2023) and providing quality family planning services - recommendations from the cdc and the us office of population affairs (2023). All these resources ensure best practices are followed at each visit.evidence-based medicine has proven that access to contraceptive supplies and services lowers the number of unintended pregnancies. Access to reproductive health services to successfully achieve a patient's wishes with regards to their reproductive life plan (number of children, birth spacing, etc.) is a human right. Before true gender equality can be achieved, women need access to comprehensive, reproductive health care. Hcc is proud to offer these services to the people in our community.

HCC FOLLOWS THE WISCONSIN DEPARTMENT OF HEALTH SERVICES WOMEN'S HEALTH-FAMILY PLANNING/REPRODUCTIVE HEALTH PROGRAM GUIDELINES (2022) AND THE WISCONSIN FAMILY PLANNING AND REPRODUCTIVE HEALTH ASSOCIATION'S REPRODUCTIVE HEALTH BEST PRACTICE GUIDELINES (2021). OUR MEDICAL TEAM ALSO WORKS UNDER THE UNITED STATES MEDICAL ELIGIBILITY CRITERIA FOR CONTRACEPTIVE USE (US MEC, 2023) AND PROVIDING QUALITY FAMILY PLANNING SERVICES - RECOMMENDATIONS FROM THE CDC AND THE US OFFICE OF POPULATION AFFAIRS (2023).
