Health care for the homeless: health care for the homeless provides access to primary health care, medical case management and substance/mental health referrals for homeless individuals and families in brown county. To qualify the person must be living in a shelter, living on street, living in a room without kitchen facilities, temporarily living with a friend or relative because they cannot afford to live alone or been served an eviction notice. The following services are provided.a. Urgent and primary health careb. Preventive health carec. Well-child physicals and immunizationsd. Hiv and std testing referralse. Pregnancy testing and referralsf. Alcohol and drug referralsg. Mental health referralsh. Help with emergency dental carei. Help with emergency eye carej. Help with finding resources in the communityk. Advocacyin the current year 1,301 patients benefited from the program.

Activity 3

Community clinic program: the community clinic program provides primary medical/health care for low-income uninsured families and individuals in a holistic manner. This is a community based program. Physicians are all volunteers. Sliding scale fee for referrals. In the current year 1,903 patients benefited from the program.
