“The Hospital is organized as a Wisconsin non-profit corporation and has been recognized by the Internal Revenue Service (IRS) as exempt from federal income taxes under Internal Revenue Code Section 501(c)(3). The Hospital is annually required to file a Return of Organization Exempt from Income Tax (Form 990) with the IRS. In addition, the Hospital is subject to income tax on net income that is derived from business activities that are unrelated to its exempt purpose, if applicable. The Hospital believes that it has appropriate support for any tax positions taken affecting its annual filing requirements, and as such, does not have any uncertain tax positions that are material to the financial statements. The Hospital would recognize future accrued interest and penalties related to unrecognized tax benefits and liabilities in income tax expense if such interest and penalties are incurred.”

| IRS990/Desc | 0 | Stoughton Hospital Association is an acute care, critical access hospital, fully accredited by The Joint Commission, licensed by the state of Wisconsin and a member of the Wisconsin Hospital Association and Rural Wisconsin Health Cooperative. The Hospital provides inpatient and outpatient services to its patients. Inpatient services include medical/surgical telemetry, intensive care, inpatient rehabilitative care (commonly referred to as Swing Bed) and geriatric psychiatry care. In 2015, there were 6,294 inpatient days. In addition to inpatient services, the Hospital provides a variety of outpatient services including emergency room care, urgent care, medical imaging (including in-house MRI services), surgical services, ambulatory infusion center, occupational health and rehabilitation services. In 2015, there were approximately 54,000 outpatient visits.To fulfill its mission of community service, the Hospital provides care to patients who meet certain criteria under its charity care policy without charge or at amounts less than its established rates. Revenue from services to these patients is recorded in the accounting system at the established rates, but the Hospital does not pursue collection of the amounts. The resulting adjustments are recorded as adjustments to patient service revenue, depending on the timing of the charity determination.The Hospital maintains records to identify and monitor the level of charity care it provides. These records include the amount of charges foregone for services and supplies furnished under its charity care policy. The amount of charges foregone were approximately $358,000 for the year ended September 30, 2015. Total direct and indirect costs related to charity care charges foregone were $152,000. Additionally, the Hospital extended discounts to uninsured patients of approximately $395,000 during the year ended September 30, 2015. Total direct and indirect costs related to the discounts extended to uninsured patients were $168,000 for the year ended September 30, 2015.In addition, the Hospital provides services to other patients under certain government-reimbursed public aid programs. Such programs pay providers amounts which are less than established charges for the services provided to the recipients, and for some services the payments are less than the cost of rendering the services provided.The Hospital also commits significant time and resources to endeavors and critical services which meet otherwise unfulfilled community needs. Many of these activities are sponsored with the knowledge that they will not be self-supporting or financially viable. |
