Activity 2

Intensive targeted case management: the itcm program is a clinic-based, wrap-around, case management program providing intensive services to individuals with severe and persistent mental illness. The static capacity of this program is 285 clients. Clients are referred to the program through the milwaukee county behavioral health division, service access to independent living (sail) program and the state department of corrections. The program is a milwaukee county behavioral health division funded program. The program is licensed under the wisconsin department of health and family services chapter 61, community mental health standards. The majority of the clients in the program receive social security disability, may be eligible for medicare or medicaid benefits, and receive full wrap-around services including psychiatry, nursing, case management, money management, housing and vocational support. Clients are seen up to 5 days per week in the clinic setting. The majority of the clients are also under the protected payeeship of the agency.

Activity 3

Parsons house: the parsons house is a community-based residential treatment facility for adult offenders under the supervision of the federal bureau of prisons or federal probation. All residents must be currently on probation or parole or federal pre-release status. The program provides a structural environment, where residents can develop the necessary skills to live in an independent crime and drug free lifestyle. Specifically, the program provides room and board, individual and group counseling, transportation and employment assistance, and twenty-four (24) hours supervision and referrals to other agencies, for services not directly provided by parsons house. The program has capacity for 40 residents. In 2011 while in the program approximately 200 offenders were served, 98% succesfully completed the program, 95% were alcohol/drug free, 99% did not commit any new crimes and 85% were employed at discharge.
