“The initial draft form 990 and all required schedules are prepared using a multi-disciplinary process which corporate finance and accounting, corporate legal, corporate compliance, and corporate marketing and communications personnel who focus initially on specific portions of the return. The completed draft form 990 and all schedules are then reviewed by this same multi-disciplinary team to ensure accuracy and integration of the individual parts and schedules. In addition, the information and schedules of the returns are sent to the corporation's outside tax consultants, crowe, llp for review and comment. Crowe, llp reviews the information and schedules and prepares and signs the final return. Comments from the multi-disciplinary team and from the auditors are incorporated into a proposed final version of form 990 and all schedules. This proposed final version is then provided via an online portal to all officers and members of the board of directors for their review prior to filing. Any appropriate changes requested by the officers and directors are then incorporated in the final form 990 and all schedules for filing.”

Patient activity and related revenues for most services were significantly impacted starting in mid-March 2020, as various policies were implemented by federal, state, and local governments in response to the COVID-19 pandemic. OSF's pandemic response plan has multiple facets and continues to evolve as the pandemic continues. Precautionary steps to enhance operational and financial flexibility, and react to the risks the COVID-19 pandemic presents to the business, included the following: * Implemented certain cost reduction initiatives; * Continued to monitor economic pressures such as supply availability and costs, as well as labor demand and costs; * Assessed the various federal stimulus options available to OSF as noted in the audited financial statements.”
“OSF is a not-for-profit corporation as described by Section 501(c)(3) of the Internal Revenue Code (the Code) and is exempt from federal income taxes on related income pursuant to Section 501(c)(3) of the Code. Under ASC Subtopic 740-10, Accounting for Uncertainty in Income Taxes - An Interpretation of FASB Statement No. 109, OSF and PCI must recognize the tax benefit from an uncertain tax position only if it is more likely than not that the tax position will be sustained on examination by the taxing authorities, based on the technical merits of the position. The tax benefits recognized in the consolidated financial statements from such a position are measured based on the largest benefit that has a greater than 50% likelihood of being realized upon ultimate settlement. As of September 30, 2021 or 2020, OSF and PCI do not have any uncertain tax positions.”
