Governance Explanations

Form 990, Part VI, Section B, Line 11B

The board reviews the 990 and approves it for filing before the return is filed with the irs.

Form 990, Part VI, Section B, Line 12C

The organization requires every board member to review and sign a commitment form or a renewal of commitment form on an annual basis.

Form 990, Part VI, Section B, Line 15A

The board chair completes an industry standard, non-profit ceo evaluation in march and it is reviewed by the executive committee in its completed form. Upon reaching consensus over the evaluation, the ceo compensation is benchmarked against other ceo positions for similar sized agencies via available resources such as industry salary surveys and guidestar. Any performance merit increase and/or bonus is discussed at the executive committee. Once a decision has been reached, the review is discussed with the ceo along with the salary recommendations. Salary recommendations are given to the human resources director and business operations director for implementation.

Form 990, Part VI, Section C, Line 19

The organization's annual report, 990, and financial statements are available on their website. All other governing documents are available on request.

Financial Statement Notes

PART X, LINE 2:

Management evaluates whether tax positions reported on returns are more likely than not to be sustained if challenged. This assessment occurs at least annually and includes, most significantly, the exemption from income tax that haven operates under. Management believes no such positions exist that would have a significant impact on the organization's financial position. As of september 30, 2021, no liability for uncertain tax benefits was recorded.
