Every board member, officer, committee chair, committee member and senior manager of the organization shall sign a conflict of interest statement that will be kept on file with the organization records. If any board member, officer, committee chair, committee member or senior manager of the organization has any direct or indirect interest in, or relationship to, any individual or organization which proposes to enter into a transaction with the organization, such person shall provide prompt written notice of such interest or relationship to the board of directors of the organization, and shall refrain from participating in any discussion or voting on that particular transaction and shall not otherwise attempt to exert any influence on the organization to affect the outcome of the decision making process.

The ceo/ed and the board have responsibility to ensure that salaries are fair and reasonable. Using appropriate market comparisons (local, regional, or national) for comparable positions in comparable organizations, seva will target salaries in the range from the 50th to the 75th percentile depending on performance and experience. All salaries must be approved by the ceo/ed or the coo. In specific cases, the ceo/ed or coo may approve salaries beyond the 75th percentile. Reasons may include specific advanced credentials, key position for strategic or growth reasoning, or inability to recruit at reported market level. Salaries in excess of the 75th percentile need to be reviewed with the executive committee. Salaries in excess of the 90th percentile need written rationale and prior approval of the executive committee. The salary of the ceo/ed is set by the executive committee using the same principles. Each year, the ceo/ed will review the salaries of the five (5) highest paid staff with the executive committee and the full board. Each employee's salary is determined at the time of hire. Adjustments are made at the discretion of the employee's supervisor in collaboration with the executive director. Cost of living adjustments (colas) if approved by the board in the annual budget will be implemented in july. New staff who are hired between march and june 30th of that year are not eligible for cola or merit increases until july of the following year.

Each year, management considers whether any material tax position the foundation has taken is more likely than not to be sustained upon examination by the applicable taxing authority. Management believes that any positions the foundation has taken are supported by substantial authority, and hence, do not need to be measured or disclosed in these financial statements.

DURING THIS REPORT YEAR, SEVA'S NETWORK OF PROGRAMS AND PARTNERS PROVIDED EYE CARE SERVICES TO 961,030 RECIPIENTS AND SIGHT RESTORING SURGERIES TO 92,792. SEVA AND ITS PARTNERS HAVE DEVELOPED A COMPREHENSIVE EVALUATION SYSTEM, WHEREBY PATIENT DATA IS CAREFULLY RECORDED AND FOLLOW-UP TREATMENT IS PROVIDED. DATA IS COLLECTED ON EACH PATIENT'S AGE, GENDER, RESIDENCE, METHOD OF REFERRAL, SYMPTOMS, DIAGNOSIS, AND TREATMENT PLAN. AT EVERY EYE CAMP AND EYE CARE CENTER, SEVA SETS PATIENT TARGETS BASED ON THE POPULATION AND LEVEL OF OUTREACH CAMPAIGNS CONDUCTED IN THE RESPECTIVE CATCHMENT AREAS. SEVA MONITORS THE NUMBER OF PATIENTS SCREENED AND THE NUMBER OF PATIENTS THAT RECEIVE SIGHT SAVING SURGERY, GLASSES, MEDICATION, OR OTHER INTERVENTIONS. OUTCOMES ARE ANALYZED ON A REGULAR BASIS. WHEN EVALUATION RESULTS DETERMINE THE NEED FOR A CHANGE IN PROGRAM DELIVERY, THESE CHANGES ARE INCORPORATED INTO FUTURE PROGRAM PLANNING, FROM SURGICAL PROTOCOL, TO TRAINING, TO OUTREACH ACTIVITIES.

WITH TRAINING, TECHNICAL EXPERTISE AND CASH RESOURCES FROM SEVA, THESE INSTITUTIONS MENTORED 65 UNDERPERFORMING HOSPITALS TO INCREASE THEIR ABILITY TO PROVIDE FREE OR LOW COST, HIGH QUALITY, SIGHT-RESTORING SURGERY AND COMPREHENSIVE EYE CARE SERVICES. THIS PAST YEAR, SEVA ADDED 14 EYE HOSPITALS TO THE NETWORK. A RECENTLY COMPLETED IMPACT STUDY OF THE FIRST 26 HOSPITALS MENTORED THROUGH THE GSI PROGRAM SHOWED THAT BY THE FOURTH YEAR OF MENTORSHIP, THESE HOSPITALS WERE PERFORMING AN AVERAGE OF 87% MORE CATARACT SURGERIES OVER BASELINE, AS WELL AS AN AVERAGE INCREASE OF 59% IN EFFECTIVENESS OF SURGEONS OVER BASELINE.

IRS990/ProgSrvcAccomActy3Grp/Desc0NEPAL SIGHT PROGRAM AND EARTHQUAKE RESPONSESEVA FOUNDATION'S STAFFING, COMMUNICATIONS, AND PARTNERSHIP INFRASTRUCTURE IN NEPAL ENABLES SEVA TO SUPPORT EYE SERVICES IN MORE THAN 20 LOCATIONS NATIONWIDE. MOST OF SEVA SIGHT SERVICES IN NEPAL ARE PROVIDED THROUGH EYE HOSPITALS AND CLINICS LOCATED IN REMOTE AREAS WHERE SEVA ALSO HAS STRONG COMMUNITY RELATIONSHIPS. THIS INFRASTRUCTURE WAS ACTIVATED IMMEDIATELY FOLLOWING THE APRIL 25, 2015 EARTHQUAKE. THROUGH CAREFUL PLANNING WITH SEVA PARTNERS INSIDE AND SURROUNDING THE QUAKE AFFECTED ZONE, SEVA DEPLOYED VALUABLE RESOURCES FOR RAPID RESPONSE, RELIEF AND REBUILDING FOR COMMUNITIES IN NEED IN COORDINATION WITH THE GOVERNMENT OF NEPAL AND RELATED AGENCIES. IN PARTNERSHIP WITH BHARATPUR EYE HOSPITAL AND LUMBINI EYE INSTITUTE, SEVA PROVIDED RELIEF IN THE FORM OF OVER 90,000 LBS OF FOOD, OVER 7,000 TENTS AND TARPS, AND OVER 1,000 BLANKETS AND MATS, TO COMMUNITIES LIVING IN GORKHA, DHADING, AND RASUWA DISTRICT. WITH INTERNATIONAL AND LOCAL PARTNERS, SEVA ALSO IMPLEMENTED REBUILDING ACTIVITIES IN SINDHUPALCHOWK, NUWAKOT, RAMECHHAP, KATHMANDU, KHUMBU, BOCHA, BORLE, AND SAHARE. REBUILDING ACTIVITIES INCLUDED: DEVELOPING, TESTING, AND BUILDING EARTHQUAKE RESILIENT HOUSING AND SCHOOLS; PROVIDING TEMPORARY LEARNING CENTERS UNTIL PERMANENT SCHOOLS ARE BUILT; AND REPAIRING AND CONSTRUCTING LOCAL INSTITUTIONS LIKE SCHOOLS, NUNNERIES, AND CLINICS.
