SERVICES ARE TARGETED TO THOSE WHO ARE FRAIL, AT NUTRITIONAL RISK, MOST ECONOMICALLY NEEDY, MINORITIES, AS WELL AS THOSE WHO ARE IN RURAL OR ISOLATED AREAS. 1,054 PARTICIPANTS WERE SERVED AND WERE PROVIDED 251,233 DAYS OF CARE COORDINATION AND HAD 184,269 UNITS OF IN-HOME SERVICES ARRANGED FOR THEM. THE ORGANIZATION PROVIDES SERVICES THROUGH OTHER CONTRACTED COMMUNITY SERVICE ORGANIZATIONS. THESE SERVICES INCLUDE INFORMATION & ASSISTANCE (10,434 SERVED), ELDER ABUSE EDUCATION (839 SERVED), HEALTH PROMOTION (377 SERVED), CAREGIVER ASSESSMENT & SUPPORT (322 SERVED), KINSHIP CARE (25 SERVED), LONG TERM CARE OMBUDSMAN SERVICE (658 SERVED), TRANSPORTATION (638 SERVED), LEGAL SERVICES (326 SERVED) AND MEDICARE/MEDICAID ASSISTANCE PROGRAM/MMAP (9,212 SERVED) |
