Prior to filing, the form 990 is presented to either the finance committee of the board or the full board for review and approval.

Form 990, Page 6, Part VI, Line 12C

Annually, the board of directors is issued the policy for review and are required to complete and sign the form indicating any conflict of interest or a lack thereof. These forms are incorporated as part of the minutes of the meeting where the review occurs. Any conflict of interest is noted by the president of the board. This person is then prohibited from any discussion or vote relevant to that matter. Any conflicts of interest on part of the executive director or key staff are reviewed on a case by case basis by the executive committee of the board at the point of occurrence. The policy is also incorporated as part of the hiring packet for staff (if appropriate) and the form is included in their employee file.

On an annual basis, the executive committee of the board undertakes a review of the compensation for the executive director. This review includes, but is not limited to, performance input from key members of the staff, a review of any available nonprofit compensation surveys publicly available, review of compensation practices and levels through consultation with board members of other similar nonprofit agencies within the community, and an analysis of budget and program performance data for the agency. The proposal of compensation for the new fiscal year is then brought to the board of directors for approval. All committees maintain a contemporaneous set of minutes of each meeting which are then incorporated into the documents approved at the next full board of directors meeting.

The determination of salary ranges for staff other than the executive director is the responsibility of the executive director and is done in conjunction with the executive committee. This determination is based on all of the resources which can be used by the executive committee in reviewing the compensation for the executive director but is also subject to the constraints of anticipated funding for the year. The full board approves these decisions by virtue of approving the proposed salary ranges and budget for the new fiscal year.

Workzone, an intensive employment training and paid internship program for homeless youth ages 18-20, prepares youth for their first job by providing employment training focused on resume-writing, interviewing, and professional workplace behavior, a 100-hour paid internship at a local business, nonprofit, or company, and case management to address homelessness and other external barriers to employment. 32 youth were provided training and stipends during 2015-16. The education fund at ozone house is available to finance a variety of educational expenses that ozone house clients incur that are not typically covered by financial aid and scholarships for books, supplies, technology, extra-curricular fees, housing deposits, et al. Having an education fund specially designated for ozone house clients helps make the goal of a continued education beyond high school possible for many young people who do not have the resources or parental support that it takes to apply to college. Three youth were given assistance during 2015-16.

The residents of the transitional housing group home are required to set up a plan to learn the life skill of paying rent. To facilitate this, each resident pays a monthly amount, based on their service plan, which is deposited to an individual account on the agency books which is held in trust until the client leaves the group home. Clients may also deposit additional monies to their account as well as receive and deposit incentive rewards for meeting performance targets.

THE EDUCATION FUND AT OZONE HOUSE IS AVAILABLE TO FINANCE A VARIETY OF EDUCATIONAL EXPENSES THAT OZONE HOUSE CLIENTS INCUR THAT ARE NOT TYPICALLY COVERED BY FINANCIAL AID AND SCHOLARSHIPS FOR BOOKS, SUPPLIES, TECHNOLOGY, EXTRA-CURRICULAR FEES, HOUSING DEPOSITS, ET AL. HAVING AN EDUCATION FUND SPECIALLY DESIGNATED FOR OZONE HOUSE CLIENTS HELPS MAKE THE GOAL OF A CONTINUED EDUCATION BEYOND HIGH SCHOOL POSSIBLE FOR MANY YOUNG PEOPLE WHO DO NOT HAVE THE RESOURCES OR PARENTAL SUPPORT THAT IT TAKES TO APPLY TO COLLEGE.
