Governance Explanations

Form 990, Part VI, Section B, line 11

The board retains the services of an independent CPA firm to prepare the organization's Form 990. The Chief Financial Officer and Executive Director review the completed Form 990 and provide a full copy to all voting members of the governing body prior to filing. The governing body is provided a reasonable amount of time to review the return and ask any questions directly to the Chief Financial Officer and Executive Director or the contact at the independent CPA firm prior to filing.

Form 990, Part VI, Section B, line 12C

Kaleidoscope monitors and enforces compliance with the conflict of interest policy. As part of the nomination process all new members are provided with an orientation packet that consists of a copy of the bylaws and the conflict of interest policy and disclosure which they are required to sign. In addition, on an annual basis they are required to update the document with their signature.

Form 990, Part VI, Section B, line 15

During the months of May and June the organization contracted with an outside firm to conduct an agency wide compensation analysis. The organization was provided with a comprehensive report that provided in depth analysis as an agency and each individual position within the agency. This analysis brought together three separate benchmarks, and provided recommendations for minimum, mid-point and maximum compensation ranges for each position. This information was provided to the Board of Directors and they based their recommendation for annual raise and compensation for the Executive Director and other officers of the organziation.

The TLP Implementation Program is a DCFS program featuring DCFS consultants hired to assist agencies in redesigning transitional and independent living programs. The Agency acts as the fiscal agent for this program. This program was discontinued in 2016.

Part X, Line 2:

Accounting principles generally accepted in the United States of America require management to evaluate tax positions taken by the Agency and recognize a tax liability if the Agency has taken an uncertain position that more likely than not would not be sustained upon examination by the IRS or other applicable taxing authorities. Management has analyzed the tax positions taken by the organization and has concluded that as of June 30, 2016 and 2015, there are no uncertain positions taken or expected to be taken that would require recognition of a liability or disclosure in the financial statements. The Agency is subject to routine audits by taxing jurisdictions; however, there are currently no audits for any tax periods in progress.

Families in this program may volunteer to receive our services, or they may be ordered by a court to participate. Our families face challenges related to poverty, domestic violence, substance abuse, mental health and housing. We provide counseling, in-home visits to monitor and assess the family, and case management in effort to prevent the removal of children from the caregivers' homes, improve the home environment for the children and transition the household out of the DCFS system.

Management has analyzed the tax positions taken by the organization and has concluded that as of June 30, 2016 and 2015, there are no uncertain positions taken or expected to be taken that would require recognition of a liability or disclosure in the financial statements. The Agency is subject to routine audits by taxing jurisdictions; however, there are currently no audits for any tax periods in progress.
