Monitoring and enforcing compliance with the conflict of interest policy is accomplished through board members signing a conflict of interest agreement annually. In addition, employees must adhere to a code of conduct document and are responsible for communicating potential conflicts of interest through the compliance line.

Additional Explanations

Form 990, Part I, Line 6

Maple lawn homes is one member of a group of four related tax-exempt organizations (see schedule r). Volunteers are tracked in total for the group and not for each individual member. There are 147 volunteers for the entire group of related tax-exempt organizatons. These volunteers provide services for the member organizations. The organization also runs a volunteer program where 455 people volunteer in the community.

Form 990, Part III, Line 3

The accounting for management services changed during 2014 to more accurately reflect the activity. Management service fee income was net against the related expenses incurred on behalf of the other organizations because it better reflects reimbursements for expenses as opposed to service fee income.

The organization's sole owner is maple lawn association of churches, inc., whose sponsor is mennonite health services (mhs) alliance. The organizations believe that a continued relationship to the churches through the sponsorship of mhs alliance is important for the ongoing values, commitments and operations of the maple lawn association of churches, inc.

Financial Statement Notes

PART V, LINE 4:

The endowment fund disbursements are restricted to expenditures such as staff scholarships for continuing education, grounds improvements, resident assistance, specific staff positions, and other discretionary disbursements as determined by the board of directors.

PART X, LINE 2:

Homes is qualified as a not-for-profit organization under internal revenue code section 501(c)(3) and is therefore exempt from the payment of income taxes, except for any unrelated business income under the provisions of section 511 of the code. Homes files a form 990 (return of organization exempt from income tax) annually in the u.s. Federal jurisdiction. With few exceptions, homes is no longer subject to u.s. Federal, state, or local income tax examinations by tax authorities for years before 2011.
