Mission

Sinai Health Systems mission is to improve the health of the individuals and communities it serves.

Sinai's mission is to improve the health of the individuals and communities it serves. Located on the west side of the city of Chicago and serving a population base of 1.5 million people in its service area ZIP codes, Sinai Health System (Sinai) consists of six member organizations. They are Mount Sinai Hospital, Holy Cross Hospital, Schwab Rehabilitation Hospital, Sinai Community Institute, Sinai Medical Group, and Sinai Urban Health Institute. Sinai has a number of clinics that are located on the west and south sides of Chicago. There is also one facility, Sinais Touhy Clinic on the north side of Chicago, which sees refugees from many nations (for example Sudan, Iraq and Burma) as well as resident Orthodox Jewish and Russian populations. With the exception of the Touhy Clinic, Sinai Health System serves primarily African-American and Latino patients.Sinais programs and community outreach are based on scientific epidemiological assessments of local neighborhoods done by Sinai Urban He

Form 990, Part VI, Line 11B: Form 990 Review Process

The 990 was initially prepared by the Finance Staff with multi disciplinary inputfrom Public Affairs, Corporate Integrity and other appropriate staff of theorganization. The 990 was then reviewed by senior finance staff and other members ofSenior Leadership. Prior to the filing, the 990 was made available via email to Finance Board Committee . The 990 form was presented as a discussion item for the Finance Board Committee where members had an opportunity for questions and comments. The 990 was also made available via email to the full Board of Directors.

Form 990, Part VI, Line 12C: Explanation of Monitoring and Enforcement of Conflicts

The Conflict of Interest Disclosure Form is completed and signed annually by all board members, employed physicians, management personnel as well as other employees who are in a position to influence purchasing decisions. In addition to the annual filing of the form, an update is required to be filed any time there has been a change. Based on the information disclosed, the Chief Integrity Officer, along with other members of management and counsel as needed, implements an appropriate conflict management plan, which may include requiring the interested party to abstain from participation in certain decisions. The information disclosed also is taken into consideration in making Board and Board Committee assignments. Further, board members are required to disclose potential conflicts relating to Board and Committee proceedings to the appropriate Board or Committee Chair for appropriate conflict management procedures, including abstention from participation in certain decisions.

Form 990, Part VI, Line 15B: Compensation Review and Approval Process for Officers and Key Employees

Sinai Health System has an Executive Compensation Committee to support the organizational performance of Sinai Health System and its related entities through the alignment of executive compensation with system strategies and programs and ensure compliance with applicable law. The voting members of the Committee are independent members of the Board of Directors. The Committee meets twice annually, or more frequently, as circumstances require. The Committee establishes detailed goals annually for the President and CEO and other executives, and reviews performance against these goals on an annual basis. The Committee annually engages an outside, independent compensation consultant to benchmark the salaries and benefits of the organization's Assistant Vice Presidents and above, as well as a few directors. Compensation is based on detailed written performance appraisals and external market data. In executive session, the Committee reviews the performance of the President and CEO, each element of compensation, data of compensation programs in effect for CEOs of comparable organizations, and conducts an annual review of CEO performance against established goals. The Committee maintains written minutes which are maintained in Executive Administration.

OTHER PROGRAM SERVICES 4: Support from the Aids Foundation of Chicago allows Sinai to provide Primary and HIV medical specialty care to uninsured clients in our Adult Infectious Disease clinic. Through the clinic, Sinai caregivers provide medical and social support case management services to all qualified HIV infected patients. This includes food vouchers, transportation bank and forth to visits, support systems, linkage into care and referral sources. Case manager case load is approximately 170 patients.

Income taxes: mount sinai, schwab,smg, hch and sci are tax-exemptorganizations under internal revenue code section 501 (c)(3) and each, as required, files a form 990 (return of organization exempt from income tax) annually. Hchp files federal and illinois forms 1120 (u.s. Corporation income tax return) annually. The corporation adopted financial accounting standards board (fasb) issued guidance for uncertainty in income taxes. This guidance prescribes a recognition threshold and measurement attribute for the consolidated financial statement recognition and measurement of a tax position taken or expected to be taken in a tax return. Examples of tax positions common to health systems include such matters as the following: the tax-exempt status of each entity, the nature, characterization and taxability of joint venture income and various positions relative to potential sources of unrelated business taxable income(ubit). Ubit is reported on form 990t, as appropriate. The benefit of a tax position isrecognized in the consolidated financial statements in the period during which, based on all available evidence, management believes that it is more likely than not that the position will be sustained upon examination, including the resolution of appeals or litigation processes, if any.tax positions are not offset or aggregated with other positions. Tax positions that meet the "more likely than not" recognition threshold are measured as the largest amount of tax benefit that is more than 50 percent likely to be realized on settlement with the applicable taxing authority. The portion of the benefits associated with tax positions taken that exceeds the amount measured as described above is reflected as a liability for unrecognized tax benefits in the consolidated balance sheet along with any associated interest and penalties that would be payable to the taxing authorities upon examination. As of june 30, 2014 and 2013, there wereno unrecognized tax benefits identified and recorded. Forms 990 and 1120 filed by the corporation are subject to examination by the internal revenue service (irs) for up to three years from the extended due date of each return. Forms 990 and 1120 filed by the corporation are no longer subject to examination for the years june 30, 2010 and prior.

IRS990/Desc0HUD Project:The intervention, entitled Helping Chicagos Westside Adults Breathe and Thrive: A Health Homes Approach to Improving Respiratory Health (HCWABT), is the first to examine the effects of a Community Health Worker (CHW)-based intervention on improving asthma management among adults. HCWABT builds off of SUHIs Helping Children Breathe and Thrive (HCBT) program, which officially ended in August 2013. The program served both adults and children with asthma living in CHA developments on the Westside, and saw significant improvements in overall asthma management and reductions in asthma triggers found in the home environment among participants. HCWABT is a three year project and officially kicked off in November of 2013. HCWABT is SUHIs eighth asthma grant, and when completed, SUHI will have been intervening to improve asthma management in some of Chicagos most vulnerable communities for over 15 years. CHWs conduct 5-6 home visits over the course of the 12-month intervention, where they provide comprehensive asthma and healthy homes education. We have completed seven months of recruitment. Methods of recruitment that have been implemented include referrals from: Sinai Emergency Department, Sinai inpatient units (hospitalizations), Sinai physicians and the Chicago Housing Authority. As of June 30, 2014 343 referrals were received. Sixty percent of these referrals came from Sinais Emergency Department and since contact information is limited to patients medical files, 52% of ED referrals were never reached. Although only 32% of referrals came from the Chicago Housing Authority, over half of active enrollees (57%) came from this referral source since participants initiated their own referral. Nearly 22% of total referrals were ineligible for HCWABT, in large part due to their home zip code being outside of the service area. A few ineligible participants were given one-time teaching sessions since they could not be enrolled in the program. As of June 30, 2014 76 adults have completed their baseline visit and are enrolled in the program. We continue to anticipate many more referrals as recruitment from several outpatient clinics and other community partner organizations in the projects service area will begin soon.Community Health Worker Interventions in Chicagos Health Care InstitutionsCommunity Benefits FY2014:Health care reform calls for innovation and systems change. Provisions of the Affordable Care Act, such as hospital readmission reduction and patient-centered medical homes, well match Community Health Worker (CHW) roles and work. As such, the utilization of CHWs as valued members of health care teams is progressively being discussed by medical providers, researchers, and policy-makers as a strategy in complying with health care reform and meeting the goals of health cares triple aim. However, there is uncertainty among health care administrators and providers as to how to implement the CHW model to obtain the triple aim- better health, better quality, and lower health care costs. CHWs have been central to SUHIs work in eliminating health disparities in Chicagos most vulnerable communities since its inception. A wealth of experience has been amassed in recruiting, training, supervising and deploying the CHW workforce to deliver health education, assist clients in navigating the health care system, and connecting clients with appropriate social service needs. SUHI has implemented CHW projects in various health conditions, including asthma management, diabetes, breast health, HIV/STD and smoking cessation.
