NATURE OF BUSINESS: THRESHOLDS HOUSING, INC.(PROJECT) IS A NONPROFIT SOCIAL SERVICE AGENCY WHICH PROVIDES HOUSING FACILITIES AND SERVICES SPECIALLY DESIGNED TO MEET THE PHYSICAL, SOCIAL AND PSYCHOLOGICAL NEEDS OF INDIVIDUALS FACING THE CHALLENGE OF RECOVERY FROM SEVERE AND PERSISTENT MENTAL ILLNESS. THE PROJECT OWNS AND OPERATES A 12-UNIT APARTMENT BUILDING IN CHICAGO, ILLINOIS KNOWN AS JACQUELINE TERRACE, WHICH IS SUBJECT TO FEDERALLY INSURED FIRST MORTGAGE LOAN OBTAINED UNDER THE PROVISIONS OF SECTION 202 OF THE NATIONAL HOUSING ACT (FHA PROJECT NO.071-EH086). THE APARTMENTS GENERALLY ARE LEASED FOR ONE-YEAR TERMS AND ARE REGULATED BY THE U.S. DEPARTMENT OF HOUSING AND URBAN DEVELOPMENT (HUD) AS TO RENTS AND OPERATING METHODS.
