Everwell's mission is to provide high quality, cost effective health care to the citizens of new hampshire and the surrounding region; to improve population health by deploying and synchronizing human and facility resources for efficient and effective patient/family management methods and strategies; to improve efficient and effective deployment of scarce resources to support education, research and training of current and future physicians and other health care providers; and to increase the value and to improve the quality of health care provided.

Members of the Organization

Form 990, Part VI, Section A, Line 2 Everwell is part of the Dartmouth-Hitchcock Health System, which owns for-profit subsidiaries that provide services and support the mission of the organizations. As a result, some officers of the Everwell are also officers of for-profit entities within the healthcare system. Form 990, Part VI, Line 6 & 7a The organization is made of member organizations supported by Everwell. There is a single class of members who shall have such rights as stated within the articles of agreement and bylaws. The members consist of Dartmouth Hitchcock-Health and Elliot Health System, both 501(c)(3) organizations. No other persons or entities are entitled to membership. Each member organization elects representatives to the board of Everwell. Members have the reserved powers to appoint trustees, appoint/discharge the CEO of the corporation, appoint administrator of joint services, change or repeal amendments to the Bylaws the are Board approved, approve Everwell's requests for any contributions from member organizations, and to fullfill obligations under state or federal licensing of the members or affiliates with respect to joint services. A copy is provided to the board for review and comments one week prior to filing. THE DARTMOUTH HITCHCOCK OFFICE OF POLICY SUPPORT, COMPLIANCE AND AUDIT SERVICES CONDUCTS AN ANNUAL SURVEY OF ALL OFFICERS AND TRUSTEES AND PERFORMS OTHER PROCEDURES AS CONSIDERED NECESSARY TO REPORT ON COMPLIANCE WITH THE CONFLICT OF INTEREST POLICY. IRS990/Desc0Everwell's purpose is to further the missions of its supported organizations through the described means as stated in Everwell's Supporting Organization Status and Mission. Fiscal Year 2015 accomplishments include: The joint Gastroenterology Service (GI) added capacity to improve patient access especially for colon cancer screening. This service, offered in two office locations, also worked to coordinate schedules to improve clinician availability for patients. The joint department continues to work with the state of NH on quality measures and the national registry for performance data collection. The GI service participates in the Manchester City program for colon cancer screening and follow-up of indigent citizens. The GI service worked with the Anesthesia Dept to develop clinical criteria that would allow certain patients to be treated in an out-patient setting rather than in-hospital. This was done across an array of procedures performed based on patient management needs. The cost of services rendered in the out-patient setting is lower than the in-patient setting so this initiative should lower the cost of care for qualifying patients. The clinicians in the joint GI service continue to support and participate in education and training for Medical students, Nursing students, and Medical Assistant students. The Endocrinology Service added capacity- both Diabetic Educator and Nurse Practitioner- to improve patient availability and enhance the Diabetic management program. The Endocrinology Service worked to coordinate care for select patients undergoing treatment for Thyroid Cancer with DHMC surgeons that now includes surgical treatment and follow-up in Manchester thereby reducing the travel time and distance barrier related to their care.
