The mission of loma linda university behavioral medicine center is to provide the community with health care in a competent & compassionate environment, & to create a clinical & research environment for the education of physicians & other health professionals in fulfilling our core mission, "to make man whole". "to make man whole" encompasses providing services to individuals in need of emotional & behavioral restoration in a positive outcome environment concurrent with the education of physicians & other health care professionals.

The mission of loma linda university behavioral medicine center is "to make man whole." this is fulfilled by providing the community with competent and compassionate behavioral health care while creating an exceptional clinical and research environment for the education of physicians and other health professionals.

The sole corporate member of llubmc pursuant to section 9310 of the california nonprofit religious corporation law shall be lluh, a california non-profit religious corporation, acting by and through its board of trustees.

Lluh, as the corporate member, through its board of trustees has reserved powers to appoint the governing board of llubmc and to remove from office any trustee of the governing board of llubmc with or without cause.

Form 990, Part VI, Section A, Line 7B

The powers reserved by lluh as member, which may only be exercised by the lluh board upon the affirmative vote of two-thirds of the trustees present and voting at any regular or special meeting of the board at which a quorum is present are to: 1. Approve or ratify all changes to the articles of incorporation of bylaws of llubmc, as the member. 2. Appoint the governing board of llubmc as the member. 3. Approve any significant change in the corporate purposes of llubmc. The powers reserved by lluh as member, which shall only be exercised by the lluh board upon a majority vote of lluh's trustees present and voting at a regular or special meeting whenever a quorum is present, are to: 1. Remove from office any trustee of the governing board of llubmc with or without cause. 2. Review and consult with the governing board of llubmc regarding the appointment of the president/chief executive officer of this entity. However, the selection, appointment and removal of such president/chief executive officer is the legal responsibility of the governing board of llubmc. 3. Approve any merger, consolidation, dissolution, sale or transfer of more than ten million dollars of the assets of llubmc. 4. Require llubmc to have a policy for borrowing funds that is congruent with the lluh board-approved policy on borrowing. 5. Approve all borrowing of five million dollars or more by llubmc or its subsidiaries (see board-approved policy for borrowing of funds). 6. Approve any exceptions to the policy for the borrowing of funds by llubmc. 7. Approve the strategic plan of llubmc. 8. Approve major construction projects of llubmc. 9. Approve institutes; approve corporations subsidiary to llubmc. 10. Accept the annual audited financial statements of llubmc. 11. Approve the annual operating and capital budgets of llubmc. 12. Approve llubmc contracting guidelines. 13. Approve recommendations from llubmc and its subsidiaries for business development plans and activities. 14. Require llubmc to have a compliance program. If lluh does not give approval or ratification of an item noted above, then the item will be returned to the governing board of llubmc for reconsideration before resubmission. Nothing in the organization's bylaws is intended, nor shall be construed, to diminish the authority of the governing board and officers of llubmc over its own strategic, operational and management issues as required by california law, accrediting bodies, and/or financing documents.

Lluh, llubmc's parent organization, employs an independent outside consulting firm to benchmark compensation for lluh employees. Llubmc's ceo and cfo are employed by lluh. The lluh executive compensation committee uses the results of the independent survey to establish pay scales for the next fiscal year. The executive compensation committee sets the total compensation below the industry mean and median amounts provided in the survey. The executive compensation committee is composed of trustees appointed by the lluh board who are independent of lluh's management. The executive compensation committee is appointed by the lluh board of trustees to have direct responsibility for establishing compensation, policies, and benefits for the senior executives of lluh. The committee's process includes contemporaneous substantiation of deliberations and decisions. For key employees other than the officers as well as employees in leadership positions, llubmc management, through the human resource department, obtains the services of an independent consulting firm to conduct nationwide compensation surveys. Management uses the comparable benchmarks in the survey results to establish or review pay scales for the position. This survey, conducted by the independent consulting firm, is performed for the organization every two years. Internally, the human resource department conducts annual surveys using at least three survey results from independent firms to establish or review the reasonableness of compensation amounts. This process was last undertaken in december of 2015 for the position of vp & administrator, llubmc held by field, edward l. Llubmc will make its governing documents and conflict of interest policy available to the public upon request and in accordance with irs guidelines. The conflict of interest policy is posted on the organization's internal website for employees. Llubmc makes its un-audited quarterly financial statements and year-end audited financial statements available to the public pursuant to its continuing disclosure agreements with bondholders.

Part VII, Line 1A

For employees of lluh or lluh affiliates (organization(s)) related to the filing organization who devote less than full-time to the filing organization (based upon the average number of hours per week shown in part vii, section a, column (b) of the return) the compensation amounts shown in columns (e) and (f) were provided in conjunction with their responsibilities and roles in other positions for organizations related to the filing organization. These individuals each devote approximately 50 hours per week serving in their respective positions within the organization.

Llubmc disclosures after the year end of december 31, 2016, llubmc underwent a thorough review of the organization's overall compliance with internal revenue code (irc) section 501(r) and final treasury regulations. During this assessment, llubmc and an external consultant reviewed the facilities' financial assistance policy (fap), billing and collections policy (b&c), plain language summary (pls), billing statements and notifications, community health needs assessment (chna), and related internal procedures. Several gaps, that were deemed to be minor during this review, were identified and are in the process of being corrected. Additional information is available upon request. Community health needs assessment pursuant to treasury regulation section 1.501(r)-3(b)(5)(i)(c), llubmc must take into account public comments received on facility's chna and implementation strategy (is). In 2016, llubmc received feedback from the community served via focus groups that were included in the 2016 chna report. The community benefit advisory council (cbac) was created in 2012; members include lluh leadership, county leadership, community-based organization leaders and population health physicians. Starting july 1, 2017, the institution created community benefit steering committee (cbsc) in addition to the cbac, to receive ongoing feedback and share the community benefit activities. The cbsc has group of community leaders, community representatives, health care professionals and public health professionals from our community. The members will contribute to loma linda university health's efforts to better understand the needs of our community; and evaluate our community benefit initiatives on an ongoing basis. The committee also has an integral role in chna and advises the organization on the is. Llubmc's community benefits team has developed a process and documentation to ensure stronger community involvement in its chna and is. Pursuant to treasury regulation section 1.501(r)-3(b)(6)(i)(f), llubmc must evaluate the impact the facility's actions have had on the community's health needs identified in the prior chna. During 2016, the process to evaluate the impact of the chna and is had not been closely monitored. Since january 1, 2017, the process has been improved to evaluate the impact of actions taken by llubmc. Pursuant to treasury regulation section 1.501(r)-3(c), llubmc must adopt an implementation strategy to describe how the hospital facility will and will not address the significant health needs as identified in the chna. The community health plan report 2017-2019 (also known as the implementation strategy) detailed the significant health needs it would address. To ensure all regulatory requirements are met, the community benefit 2016 report, community health needs assessment 2016 report and community health plan 2017-2019 report, were audited by external agency and legal advisors. All general guidelines and requirements were met for the institution. However, the report did not clearly include the anticipated impact of the actions, the resources to be committed to addressing the needs, and all the collaboration efforts it intends to take with other organizations. Llubmc also did not clearly list and rationalize the significant health needs they would not be addressing. On november15, 2017, llubmc published a revised community health implementation strategy 2017-2019 that was adopted by the organization. Llubmc's community benefit reporting team has updated its internal processes to ensure future reports meet the requirements.

ALTHOUGH REIMBURSEMENT FOR SERVICES IS CRITICAL TO THE OPERATION AND FISCAL STABILITY OF LLUBMC, IT IS RECOGNIZED THAT NOT ALL INDIVIDUALS POSSESS THE ABILITY TO PURCHASE ESSENTIAL MEDICAL SERVICES. FURTHER, LLUBMC'S MOTTO "TO MAKE MAN WHOLE" REQUIRES IT TO PROVIDE HEALTHCARE SERVICES AND EDUCATION TO THE ENTIRE COMMUNITY. IN KEEPING WITH LLUBMC'S COMMITMENT TO SERVE ALL MEMBERS OF THE COMMUNITY, MANY SERVICES--FREE CARE, SUBSIDIZED CARE, CARE PROVIDED AT OR BELOW COST TO PERSON COVERED BY GOVERNMENTAL PROGRAMS AND FREE EDUCATION SERVICES--WILL BE CONSIDERED WHERE THE NEED AND THE INDIVIDUALS INABILITY TO PAY COEXIST. AS SUCH IN 2016, LLUBMC HAS PROVIDED $2,537,263 IN PATIENT CHARITY CARE. OTHER CHARITABLE ACTIVITIES INCLUDE, BUT ARE NOT LIMITED TO, COMMUNITY EDUCATION PROGRAMS, FREE ON-SITE SELF-HELP GROUPS (I.E. ALCOHOLIC ANONYMOUS), COMMUNITY OUTREACH PROGRAMS (I.E. HEALTH FAIRS), A FREE 24 HOUR CRISIS HELP-LINE, FREE ON-SITE MENTAL HEALTH AND SUBSTANCE ABUSE EVALUATIONS, FREE COMMUNITY REFERRALS AND SUPPORT ACTIVITIES FOR LOCAL SCHOOLS AND CHURCHES. IN 2016, 47,802 COMMUNITY MEMBERS WERE PROVIDED EDUCATION REGARDING MENTAL HEALTH ISSUES AND PROVIDED RESOURCES IN VENUES SUCH AS EDUCATIONAL FORUMS, HEALTH FAIRS, NEWSLETTERS OR LECTURES. BELOW ARE MORE DETAILED EXAMPLES TO THE COMMUNITY BENEFITS THAT WE OFFER AS IT RELATES TO OUR PATIENT CARE.ADULT PARTIAL HOSPITALIZATION PROGRAM- THE PARTIAL HOSPITALIZATION PROGRAM ALLOWS PATIENTS TO RECEIVE TREATMENT THROUGHOUT THE DAY AT A LLUBMC FACILITY AND THEN RETURN HOME IN THE EVENING. THESE PROGRAMS AIM TO PREVENT INPATIENT HOSPITALIZATION AND ASSIST PATIENTS ACHIEVE INDEPENDENCE. APPROXIMATELY $250 WORTH OF VARIOUS COLLATERAL PIECES SUCH AS BROCHURES, FLYERS AND OTHER PROMOTIONAL ITEMS WERE DISTRIBUTED FOR THIS SERVICE. SCREENINGS ARE TYPICALLY GEARED TOWARDS THE GENERAL COMMUNITY IN THE INLAND EMPIRE, SENIOR FACILITIES, AND/OR EMPLOYER ORGANIZATIONS. AT LEAST ONE CLINICAL THERAPIST OR PROGRAM REPRESENTATIVE HANDLES PROGRAM SPECIFIC QUESTIONS AND INTERPRETS DEPRESSION SCREENING AND MENTAL HEALTH ASSESSMENT RESULTS. IN ADDITION 247 INDIVIDUALS SCREENING WERE CONDUCTED AT EVENTS IN PARTNERSHIP WITH YOUTH SERVICES. SUBSTANCE USE RECOVERY- SUPPORT AND SERVICES OFFERED TO THE RECOVERING COMMUNITY INCLUDE: PROVIDING ROOM SPACE (20X20) AND COFFEE FOR COMMUNITY SUPPORT GROUPS (ALCOHOLICS ANONYMOUS (ENTIRE YEAR), NARCOTICS ANONYMOUS (THROUGH - DECEMBER 2014), AA WOMEN'S GROUP (ENTIRE YEAR) TO CARRY OUT THEIR MEETINGS WITH ANNUAL ATTENDANCE OF 833. ATTENDANCE FOR ALL OF THE SUPPORT GROUPS AVERAGED ABOUT 25,900 FOR THE YEAR. A COMPLIMENTARY NEWSLETTER WENT TO 3,667 RECIPIENTS OF THE RECOVERING COMMUNITY 2X IN 2016. THE COST FOR THIS SERVICE AMOUNTS TO $6,390. OTHER COMMUNITY BENEFITS INCLUDE (2) GRADUATIONS WITH A TOTAL ATTENDANCE OF 150 WITH 45 STAFF HOURS, 76 RAA MEETINGS WITH 25 STAFF HOURS, 130 PARTICIPANTS AT DRUG COURT GRADUATIONS. YOUTH SERVICES- PRIMARY ACTIVITIES INVOLVED FOR THIS GROUP CONSISTED OF HEALTH EDUCATION SERVICES AND TRAINING GEARED TOWARDS COMMUNITY MEMBERS, LAW ENFORCEMENT, MEDICAL PROVIDERS, FAITH BASED LEADERS AND SCHOOL PERSONNEL IN HOW TO HANDLE DIFFICULT SITUATIONS WITH CHILDREN EXHIBITING BEHAVIORS. A TOTAL OF 1,481 COMMUNITY MEMBER WERE REACHED IN 2016 WITH 64 STAFF HOURS. APPROXIMATELY $1,800 WORTH OF COLLATERAL WAS DISTRIBUTED AND OVER 32.5 HOURS OF TRAINING ON SELF-INJURY WAS PROVIDED TO 230 COMMUNITY PROFESSIONALS AND CLINICIANS. APPROXIMATELY 782 COMMUNITY MEMBERS, STUDENTS AND PROFESSIONALS WERE EDUCATED ABOUT THE MEND PROGRAM THAT DEALS WITH YOUTH FACING A MEDICAL ILLNESS OR TREATMENT SUCH AS DIABETES, TRANSPLANTATION OR CHEMOTHERAPY THAT AFFECTS THEIR EMOTIONAL HEALTH.
