Mission and Program Overview

Mission

The Camden Coalition of Healthcare Providers exists to improve the health of Camden residents by enhancing the quality, capacity, coordination, efficiency, and accessibility of the healthcare system. Camden, NJ is a city of 77,000 residents that is plagued by some of the nation's highest rates of poverty and violence. One percent of patients in Camden account for nearly 30% of city residents' hospital costs. As a coalition of healthcare providers, community partners, and advocates, CCHP is committed to elevating the health of patients facing the most challenging medical and social complexities within the city. CCHP innovates and tests healthcare delivery models, including care coordination and enhanced primary care practices. CCHP believes that data, engagement, and redesign are essential to revitalizing primary care and improving the healthcare system. Based on its learning on the frontlines of healthcare delivery, CCHP supports sound healthcare policies and advancements in the field

Our mission is to improve the health status of all camden residents, by increasing the capacity, qualtiy, and access of care in the city.

Financial Statement Notes

Fin 48

CCHP is exempt from Federal income taxes under Section 501(c)(3) of the Internal Revenue Code. Accordingly, the financial statements do not reflect a provision for Federal income taxes. CCHP adopted the pronouncement related to income taxes effective January 1, 2011 and there were no uncertain tax positions at the date of adoption or at December 31, 2014. There are no open tax years prior to 2011. In addition, CCHP had no income tax related penalties or interest for the year reported in these financial statements.
