Mission

LifeCare Alliance's mission is to lead the Community in identifying and delivering health and nutrition services to meet the community's changing needs. In 2014, LifeCare Alliance served nearly 15,000 clients with a demographic profile as follows: 72% female, 43% minority, 60% were age 65 or older, and 64% had an annual income of less than $20,000.

Major Activities

Activity 2
Wellness Service provides health care services in community wellness centers throughout Franklin County. The community wellness centers are located in areas where the population is underserved, has limited access to health care and/or has limited financial resources. Services include, but are not limited to, health assessments, skilled nursing care, and screening tests with a focus on health promotion and education, disease prevention and chronic disease management. In addition to registered nurses, the wellness program employs registered dietitians who provide medical nutrition therapy, weight management programs, diabetes self-management and educational programs on a wide variety of nutrition topics. Registered nurses assist clients with medication management. In 2012, the nurses and dietitians provided services to 1,178 clients in community wellness centers, and screening services to 1,069 clients in corporate and health fair settings, and had 6,671 visits to wellness and foot care centers. The adult immunization program offers a full array of adult immunizations from A(hepatitis A) to Z(zoster/shingles), as well as an international travel health clinic, which provides a comprehensive pre-travel assessment and all vaccines needed for international travel. In 2012, the nurses conducted 1,925 flu shots and 497 adult immunizations. Ninety-three percent (93%) of the Wellness Services clients are over the age of 65, 37% have annual incomes of less than $20,000, 65% are female, and 31% are minority. The wellness team also provides health care services in the corporate setting. The Catch the Wellness Spirit Wellness Program provides on-site services which include all of the same services provided in the community setting, as well as personal face-to-face coaching at the worksite with the registered nurse and dietitian. Health fairs for community organizations and businesses are a recent addition to the wellness program. In 2012, the Corporate Wellness program served 28 corporations including White Castle System, Inc., Highlights for Children, Grange Insurance, Edison Welding, and GFS Chemical. Visiting Nurses provide health care linkage service to assist patients with managing their chronic health conditions. This program, which began in 1898, also provides a Visiting Pharmacist, who assists clients in arranging medications to prevent and reduce drug interactions.

Columbus Cancer Clinic, a Medicare certified service, provides education about cancer prevention and early detection, head-to-toe cancer screenings, examinations and mammograms, regardless of a client's ability to pay. In 2012, the program served 3,135 clients providing 1,724 mammograms, 756 head-to-toe cancer screenings, and 655 clients with home care support services. In addition, the program provides low-income, underinsured or uninsured individuals living with active cancer with medical supplies, medical equipment, medication assistance, transportation to and from cancer-related medical appointments, pantry items, nutritional supplements and emergency financial assistance to home care support patients who are about to lose daily necessities, such as housing and/or utilities. The program will provide skilled nursing care, home health aide, social work, therapy and/or a dietitian if needed to keep the patient in the home setting. All home care support services are free of charge. In 2012, the Home Care Support service served 655 Central Ohioans living with active cancer. Wigs and breast prostheses may be obtained free of charge. Eighteen percent (18%) of all Columbus Cancer Clinic clients are over the age of 65, 72% have annual incomes of less than $20,000, 83% are female, and 45% are minority. The Columbus Cancer Clinic began in 1921 and is the oldest free cancer clinic in the United States. The Columbus Cancer Clinic merged into LifeCare Alliance in 2005.

The Help-at-Home staff works with a network of community contacts to assure appropriate client services are accessed. In addition, Help-at-Home coordinates home repair projects for clients in need of safety and health-related repairs, such as repairing electrical or plumbing issues, installing grab bars and railings, and repairing floors and drywall. In 2012, the program provided 12,000 hours of homemaker and/or home health aide services to 500 clients. Ninety five percent (95%) of the Help-at-Home clients are age 65 or older, 75% have incomes less than $20,000 annually, 82% are female and 37% are minority.
IRS990/ActivityOther/Description1Project OpenHand-Columbus provides home-delivered nutritionally enriched meals, pantry items, nutritional supplements, nutritional assessments and counseling and a congregate meal program to men, women, and children living with HIV/AIDS in Central Ohio. In 2012, Project OpenHand-Columbus provided more than 21,000 home-delivered meals and served more than 1,500 congregate meals, and provided more than 10,800 bags of groceries to more than 600 clients. Nearly 33% of the Project OpenHand-Columbus clients are aged 55 or over, 91% have annual incomes of less than $20,000, 16% are female, and 59% are minority. Project OpenHand began in San Francisco, and was founded in Columbus in 1994. This agency merged into LifeCare Alliance in 2004.

Catering is one of LifeCare Alliance's social entrepreneurship programs, offering full-service catering using the culinary expertise of LifeCare Alliance kitchen staff and working from the kitchen facility at 1699 West Mound Street. L.A. Catering is a preferred caterer at 16 locations throughout Central Ohio; or we can provide for your catering needs at your own facility or at our L.A. Catering Event Center. Carrie's Cafe is not just for our senior clients. The public, our donors and volunteers are all able and encouraged to take advantage of the services housed in Carrie's Cafe. This enables Carrie's Café to be an intergenerational experience. Anyone can visit the café for breakfast or lunch, use L.A. Catering for their events, rent event space, and/or take advantage of the café's programming. Like any community, the more diverse the population, the more fruitful the outcomes. Recent trends in not-for-profit agencies indicate there are a greater number of charitable organizations that recognize the need to offset annual operating expenses through social entrepreneurship. LifeCare Alliance recognizes the need for self-sufficiency and is committed to further developing a stream of compatible fee-based programs to increase agency revenue to continue to assist all those in need.
IRS990/ActivityOther/Description3Groceries-to-Go food pantry provides groceries, personal hygiene items, and cleaning supplies to individuals of the Project OpenHand-Columbus and the Columbus Cancer Clinic programs. These clients are living with HIV/AIDS and/or active cancer. In 2012, the program served 938 clients at more than 7,200 pantry visits, providing more than 21,600 grocery bags equating to 65, 000 meals, along with 1,250 cases of Ensure. IMPACT Safety has been serving the community for 20 years by developing and teaching interpersonal safety skills to a wide range of populations including women, youth, those with disabilities, the elderly, and professionals in the workplace. IMPACT Safety's expertise is recognized internationally as well as locally. Senior Pet Care provides clients assistance in taking care of their pets while still maintaining the client's ability to remain in their own home. The program assists with pet care and welfare, delivering pet food and setting up veterinary visits in the home as well as at veterinarians' offices. In 2012, LifeCare Alliance developed the AniMeals program, which implemented coordinated routes that deliver pet food directly to clients' homes who are medically challenged, homebound, or living with a disability. In 2012, the program provided food, supplies, grooming, and veterinary visits to 400 clients and 470 pets. Nearly 70% of LifeCare Alliance clients own a companion pet.

IRS990/Description0Meals-on-Wheels provides nourishing food and a daily visit to homebound, older adults and individuals with a disability and/or medical challenge 365 days a year. Meal options include hot, cold, frozen, diet, kosher, vegetarian, pureed and mechanical soft, along with meals for specialty diets. The Meals-on-Wheels program serves as many as 6,000 meals a day, including meals for kids in day care and after-school programs. In 2012, volunteers and staff delivered 1,104,873 meals in Franklin and Madison counties with volunteers contributing to 5,000 clients. Meals-for-Kids Program delivers meals to after-school care sites for school-age children and to daycare centers and charter schools. In 2012, Meals-for-Kids provided 148,050 meals to predominately lower-income and at-risk children at 14 sites in Central Ohio. Ninety percent (90%) of the Meals-on-Wheels clients are over the age of 65, 99% have annual incomes of less than $20,000, 64% are female, and 39% are minority. In 2012, 5,165 Meals-on-Wheels volunteers logged 413,989 miles and 102,021 hours of service in Franklin and Madison counties. Senior Dining Centers provide meals for older adults and individuals with a disability and/or medical challenge at community dining centers and restaurants in more than 19 locations across Central Ohio, including culturally diverse meals at 11 Asian and Somali restaurants. Transportation is available to most locations. LifeCare Alliance is the largest provider of senior meals through the community dining centers in both Central Ohio and the state, according to the Ohio Department of Aging (2008). Dining centers offer socialization, enriching programming and a nutritious meal, with the purpose of increasing the number of Central Ohioans who achieve a higher level of successful aging, which is defined as the avoidance of disease and disability, maintenance of high cognitive and physical functioning, and engagement with life. In 2012, the program served over 150,000 meals to 4,062 clients. Eighty-two percent (82%) of the Senior Dining Center clients are over the age of 65, 90% have annual incomes of less than $20,000, 68% are female, and 57% are minority.
