The Miami Valley Hospital Foundation (MVHF) makes every effort to follow the intent and wishes of all its donors. The intended uses of the organization's endowment funds and/or earnings from endowment funds are driven by the wishes/restrictions placed on the funds by the donor. A Permanent endowment results when a donation is received with a restriction that the principal or gift amount is to be retained in perpetuity and cannot be spent. The earnings from the permanent endowment are used for the purpose specified by the donor. Temporary endowments are funds in which all or part of the principal may be expended only after the expiration of a stated period of time or occurrence of a specified event, depending on the donor's wishes. Earnings from temporarily restricted endowments can be either unrestricted or restricted. The MVHF uses the temporarily restricted endowments for the intended purpose set up by the donor, only after the period of time or specified event set by the donor has been met. The categories of permanently and temporarily restricted endowments are as follows: fund various departments of Miami Valley Hospital, including Maternity, NICU and Trauma Surgery (23%); Hospital Operations (15%); Education (4%), research (2%), and miscellaneous Miami Valley Hospital operations (56%). SCHEDULE D, PART X, LINE 2: The following footnote is from the audited combined financial statements for Premier Health and affiliates. MedAmerica Health Systems (MAHS) is the parent organization of Miami Valley Hospital Foundation (MVHF). PREMIER HEALTH (PREMIER), MEDAMERICA HEALTH SYSTEMS (MAHS), ATRIUM HEALTH SYSTEM (AHS), SAMARITAN HEALTH PARTNERS (SHP) AND UVMC HAVE BEEN DETERMINED BY THE INTERNAL REVENUE SERVICE TO BE TAX-EXEMPT NONPROFIT CORPORATIONS AND PUBLIC CHARITIES UNDER SECTIONS 501(C)(3) AND 509(A), RESPECTIVELY, OF THE INTERNAL REVENUE CODE. IRS990/Desc0Through the hospital's home health provider, Miami Valley Hospital Foundation provided funding for an Advanced Illness Management (AIM) Health Coach to provide education and care coordination to improve patient self-management of chronic illness. Additionally, the Foundation provided a comprehensive program for COPD patients with an emphasis on Respiratory Therapy Intervention, to help patients and their caregivers better understand how to manage their disease at home. In 2017, the AIM program served 258 patients resulting in a 45.35% reduction in 30-day readmissions, and a 28.33% reduction in total encounters. (Continued on Schedule O)
