Hospice puts the family and patient at the center of care, and by working with physicians and caregivers, provides compassionate care for individuals nearing the end of life.

THE SERVICES INCLUDE A 24-HOUR NURSING SUPPORT FOR PAIN AND SYMPTOM MANAGEMENT, SOCIAL WORKERS TO COORDINATE COMMUNITY RESOURCES, PROVIDE EMOTIONAL SUPPORT AND ASSISTANCE WITH INSURANCE AND BEREAVEMENT CARE, CHAPLAINS TO ASSIST THE PATIENTS AND FAMILIES WITH SPIRITUAL CONCERNS, NURSING ASSISTANTS FOR PERSONAL CARE NEEDS AND LIGHT HOUSEKEEPING, A PHARMACIST TO HELP ADDRESS MEDICATION NEEDS, MEDICATIONS, MEDICAL EQUIPMENT AND SUPPLIES DELIVERED TO PATIENT'S HOME AND OVERSIGHT OF CARE BY PATIENT'S PHYSICIAN AND A HOSPICE MEDICAL DIRECTOR.
