“In august 2013, the board voted to allow the terms of the directors to be extended to three years.”
“The board has appointed the finance committee to assist in preparing the tax form 990. The finance committee is chaired by the treasurer and is made up of board members and staff who have expertise in finance. The committee approves the 990 for filing before the due date. At the annual meeting each board member receives a copy of the 990 for review. If upon review any issues or discrepancies are found, an amended 990 is prepared for board review and approval and filed with the irs.”
“The praxis project has a conflict of interest policy. Each employee is required to complete a conflict of interest statement identifying any area that they think may be a conflict as defined in the policy. If employees have influence on transactions involving purchases, contract, or leases they are required to disclose to the executive director or board of directors as soon as possible the existence of any actual or potential conflict of interest so that safeguards can be established to protect all parties. Each employee is required to update their conflict of interest statement if any changes occur during their employment with the praxis project. In addition to the employees updating their personnel files with changes, praxis monitors and enforces compliance with the policy to ensure those employees or board members who have direct access to contractual agreements and financial disclosure update their forms regularly. This includes exclusively the board chair, the treasurer, the executive director and the finance director.”

GRANTS ARE AWARDED FOR A THREE YEAR PERIOD. THE GRANTS ARE MONITORED BY STAFF AND AN INDEPENDENT, EXTERNAL EVALUATOR VIA QUARTERLY REPORTS, MEETINGS, INTERVIEWS AND PROGRESS ALONG WORKPLAN OBJECTIVES ESTABLISHED IN GRANTEE APPLICATIONS. IN ADDITION, GRANTEES SUBMIT ANNUAL FINANCIAL REPORTS AS A CONDITION FOR CONTINUED FUNDING. |
