“The form 990 is completed and then reviewed by the finance committee for approval by the board of directors. The full board reviews and approves the form 990. The board president signs the document and approves electronic submission to the irs.”
“Annually, the executive director will ensure that all staff complete a conflict of interest form identifying any relationships, positions or circumstances that may result in a conflict of interest. Similarly, the president of the board ensures that all board members complete a conflict of interest form. All forms are maintained in the corporate office.”
“The board of directors has a policy and procedure for the annual review of the executive director's compensation. Comparable data is retrieved from the minnesota council of nonprofits annual salary survey data as the board considers compensation.”
“The organization's governing documents, conflict of interest policy and financial statements are made available to the public upon request.”
“In mid-2016, RHI received a grant from the Otto Bremer Trust. The focus of this grant was to work with health care systems and clinics in outstate Minnesota to provide institutional assessments and trainings to assist in the implementation of the Standards of Inclusion and to continue to build the eLearning modules in order to increase access for outstate clinics and their staff. During 2016, RHI began planning for their first annual, national LGBTQ health conference. The Opportunity Conference: Advancing Health Equity is planned for February 27-28, 2017. RHI contracted with an event planning firm and PR contractor to assist with planning and promotions. Representatives of a variety of LGBTQ organizations and health care providers participated in the program planning for the conference. By the end of 2016, RHI had received a significant number of sponsorship commitments, far exceeding expectations and attesting to the importance of the topics to health care and related organizations.”

| IRS990/Desc | 0 | Blue Cross-Blue Shield Center for Prevention funds Rainbow Health Initiative's Equity in Action program. This grant supports the bulk of RHI's advocacy work and provides some support for the Voices of Health survey and RHI's education program. Rainbow Health Initiative publishes the Voices of Health data report and uses the data to support our advocacy work to advance policies with businesses, organizations and events that support healthy eating, active living and tobacco free spaces and events. RHI also advocates for the inclusion of LGBTQ data in public health programs and the expansion of data collection to include sexual orientation and gender identity. RHI's education programs provide cultural competency trainings on LGBTQ health issues and data for health care providers, organizations serving members of the LGBTQ community, educators, and school systems. Our on-line provider directory helps individuals find culturally competent health care providers. |

| IRS990/ProgSrvcAccomActy2Grp/Desc | 0 | MN Department of Health funds RHI's ShiftMN program. Corporate tobacco targets the LGBTQ community and youth with their advertising and corporate sponsorship support. ShiftMN provides leadership training for LGBTQ young adults. In 2016, the 4 Coordinators continued in their roles as educators - educating their peers on the impact of corporate tobacco practices and advocates working to create tobacco free spaces and including tobacco conversations as part of broader health and wellness events. During 2016, the Coordinators created and promoted their new media campaign, #5ways5minutes which was posted on buses and LRT equipment in mid-2016. Increased calls to the Tobacco Quitline from LGBTQ smokers attest to the impact of this campaign. In late 2016, RHI was granted a new 5 year contract with the MN Department of Health, Tobacco Free Communities program to support the Shift MN program. |

During 2016, RHI continued to partner with Out and Sober, to host chemical free events and skills development trainings. The toolkits continue to be a useful introduction to LGBTQ terms and health disparities for CD treatment programs and an introduction to the impact of alcohol on members of the LGBTQ communities for organizations that serve our communities. RHI continues to use these two toolkits to create safe space policies for LGBTQ individuals who are in recovery. |

The majority of this work was completed in 2015 and 2016. The number of providers listed on the provider directory continues to grow, and has resulted in a request from health care professionals for RHI to facilitate a professional network. Plans for funding and supporting this program are in process for 2017. The number of individuals who refer to the Patient Toolkit also continues to grow. The platform for the Toolkit will be updated in 2017.In 2015 RHI received a Bush Foundation Community Innovation Grant. This 18 month grant was extended to 24 months. It supports the creation of the MN LGBTQ Standards of Inclusion for Health and Human Service providers. That project included the creation of the Standards as well as a strategy for implementation. In 2016 the Advisory Board of health and human service providers completed the review and completion of the MN LGBTQ Standards of Inclusion for Health and Human Service Providers. These Standards will be the foundation for an education and training program for health and human service providers and systems. To complement the Standards, in 2016 RHI began creating a series of eLearning modules to help providers develop understandings and the skills needed as a foundation for implementing the Standards in their systems. This work will continue with the release of the eLearning modules in 2017.In mid-2015, RHI and Dr. Abel Knochel of the University of MN-Duluth received a grant from the University of MN-Clinical and Translational Studies program to support a transgender aging research project. This project was extended to 24 months. The focus is to better understand the health issues, needs and gaps in services as identified by transgender seniors and health care providers. Throughout 2016, Dr. Knochel and RHI staff conducted personal interviews with transgender seniors in the Metro and Arrowhead areas of Minnesota. A survey of providers in those two regions was also conducted. In mid-2016, RHI received a grant from the Otto Bremer Trust. The focus of this grant was to work with health care systems and clinics in outstate Minnesota to provide institutional assessments and trainings to assist in the implementation of the Standards of Inclusion and to continue to build the eLearning modules in order to increase access for outstate clinics and their staff.During 2016, RHI began planning for their first annual, national LGBTQ health conference. The Opportunity Conference: Advancing Health Equity is planned for February 27-28, 2017. RHI contracted with an event planning firm and PR contractor to assist with planning and promotions. Representatives of a variety of LGBTQ organizations and health care providers participated in the program planning for the conference. By the end of 2016, RHI had received a significant number of sponsorship commitments, far exceeding expectations and attesting to the importance of the topics to health care and related organizations. |
