Line 11b Explanation - The 990 is provided to the board for their review and approval before it is filed.

Form 990, Part VI, Section B, line 12C

RHIC has established a conflict of interest policy. All board members, officers and committee members complete an annual conflict of interest statement providing actual or possible conflicts of interest. Any member or officer with an identified conflict will remove themselves from any discussion and vote relating to the matter. In situations of possible conflicts, the determination requires two-thirds vote of the board of directors for the matter to be considered a conflict of interest. On an annual basis, the Executive Director, at the direction of the board of directors, will obtain a conflict of interest statement from all board and committee members and officers, and accumulate all known and possible conflicts of interests.

This was accomplished through the following:Developed patient safety and quality initiatives and had involvement in national and regional simulations. Continued the contract with the Indiana Hospital Association for training and education. Additionally, contracted with local hospitals to provide training services.Developed trauma training program through a two year contract with the Indiana Trauma Commission. All board members, officers and committee members complete an annual conflict of interest statement providing actual or possible conflicts of interest. Any member or officer with an identified conflict will remove themselves from any discussion and vote relating to the matter.
