Voting members review the complete 990. They vote to submit or amend upon first visit. After requested amendments are made, voting members review again and vote on final submission. All reviews, votes and copies of the 990 are attached to the board minutes.

Conflict of interest policy compliance Part VI line 12C

At the annual board meeting, all board members fill out a new conflict of interest exposure form. These forms are reviewed, any conflicts are exposed, and all signed forms are kept in board permanent records. Then, at each board meeting, any new conflict of interest disclosures are reviewed for justification and approval. If approved, disclosures are read into the minutes. Conflict of interest disclosure forms are then completed and signed by board members and submitted as attachments to the minutes. Conflict of interest disclosure forms are also kept in aggregate in the master board records.
