The form 990 is then provided to the full board of directors for their review prior to filing with the irs.”
“All officers, directors, trustees, key employees and highest paid employees (interested persons) of the organization have a duty to avoid conflicts of interest, both real and perceived, which may negatively impact the organization or those it serves. The organization's interested persons are to be guided by the organization's mission, vision and values and to serve patients, families and the general public without need for any personal favor or gain. The organization's ethics and compliance plan emphasizes the duty interested persons have to disclose any actual or potential conflicts of interest that may benefit their private interests or result in a possible excess benefit transaction. Conflicts are disclosed annually on a conflict of interest questionnaire that is distributed to the officers, directors, key employees, and highest compensated employees. In the event of any actual or potential conflicts of interest, interested persons must disclose the existence of their financial interest and disclose all material facts to the board chair, ceo or other designated persons. If it is determined an actual conflict of interest exists between the organization and an interested person, the party with a conflict of interest must abstain from any discussion or voting on the transaction or arrangement involving the conflict of interest. At each board meeting, board members are reminded that they have signed a conflict of interest disclosure and are asked to review the agenda and, for the record, disclose any items with which they may have a conflict of interest.”

“(continued from part iii) hospice of the florida suncoast hospice services - founded 36 years ago by volunteers, suncoast hospice's mission of care focuses on persons with chronic or serious illnesses & extends to those acting as caregivers or working through grief or trauma. Suncoast hospice is recognized nationwide for its innovative service delivery, advocacy for social change, technological development in the area of patient records, as well as other technology-related innovations. In addition to providing care in private homes and apartments, suncoast hospice serves those who reside in nursing homes, assisted living facilities (alfs) its own care centers, hospitals & other settings. Of those who died in pinellas county last year, approximately 70% were served by suncoast hospice. Suncoast hospice serves all people without regard to diagnoses, age, religious, ethnic or financial circumstances. Approximately $2.1 million in uncompensated care was provided to individuals & families last year. Last year, suncoast hospice served more than 8,200 patients by approximately 1,200 staff members for a total of more than 515,000 patient days. Suncoast hospice is holistic in its approach, helping to guide patients & families and supporting them in their decision making. Despite growth in stature and size, suncoast hospice remains a community organization with its leadership & staff ever mindful of what its community wants in a hospice provider. Because of its strong community connection, suncoast hospice has always had vibrant volunteer programs & activities. Volunteers, now 2,700 strong, bring a rich array of ideas, talent & experience to the organization. Last year alone, volunteers contributed 246,479 hours of service in support of suncoast's mission. Suncoast hospice relies on a solid team concept in its service delivery. Patient & family care teams consist of physicians, nurses, aides, physical therapists, nurse practitioners, chaplains, social workers & volunteers. At the center of each team are the patients and their families who may choose to accept or decline various care options open to them. Patient and family care teams may include physical & occupational therapists & palliative arts specialists and such options as massage, therapeutic touch, pet therapy, reiki, humor therapy & more. Patients may choose to avail themselves of the services offered according to their own needs, wants and belief systems. Families often remark that the patient & family care teams fit remarkably well into their homes and lives. Many have said that care is more than just a word to suncoast hospice staff & volunteers, noting the willingness to go the extra mile on behalf of patients and families. As the husband of one patient said of suncoast hospice - you always find a way to say yes. Care and service may be covered by medicare, medicaid, insurance providers or private funds. No one in need of end-of-life care is turned away. The suncoast hospice care centers, also known as inpatient centers, are places of refuge for our patients when they suffer severe pain or experience acute symptoms that cannot be resolved or managed in their own homes. Care with compassion is provided at these three centers. Each suncoast hospice care center is staffed by expert medical teams available 24 hours a day 7 days a week in settings designed expressly for the comfort & convenience of our patients and their families. Our patients stay in suncoast hospice care centers until their pain & symptoms are resolved and controlled enough to allow them to comfortably return to their residences - their chosen activities - their lives. Each center has its own distinct environment & setting but shares a common goal of creating a sense of comfort, peace & tranquility. Partners with suncoast hospice, physicians, elder care counselors and concerned citizens to carry out its mission. Programs are focused on helping individuals communicate and implement their end-of-life medical treatment decisions. Empath choices for care, inc. Offers respecting choices training courses both internally with suncoast hospice staff & externally with local care partners. Students complete 6 on-line modules & participate in a full day of classroom instruction. Number of students - 92 number of living wills - 2638 suncoast pace suncoast pace (program of the all-inclusive care for the elderly) provides a full range of home and health care services to seniors 55 years of age or older. Pace allows its participants to continue to live in their homes and maintain their independence. The program offers primary & specialty medical care, adult day center services, hospitalization & emergency care, nutritious meals, in-home support and care, physical occupational & speech therapies, transportation and pharmacy services. In addition to medical services and therapies, there are numerous activities and opportunities to socialize. Its goal is to keep the men and women it serves living safely in their own homes or together with their families for as long as possible, maintaining independence and quality of life. At september 30, 2014 179 participants are being served by this program. Aids service association of pinellas, inc. (asap) asap provides hiv education, prevention services, testing, case management, food and personal needs pantry, children's program, some financial assistance and more. Asap clients are served on an individual basis, with guidance on programs and services that are available to them. Suncoast hospice foundation seeks to foster relationships to build community investments in order to provide resources to fund empath health, inc. And its family of programs. Empath health, inc. Is a not-for-profit, integrated network of care encompassing hospice, palliative home health and physician services, assistance for independent living and daycare for seniors, services for those infected or affected by hiv, and advance care planning. Members receiving support through suncoast hospice foundation include suncoast hospice, suncoast pace, empath choices for care, inc. Aids service association of pinellas (asap), suncoast hospice institute and other programs and services of empath health. Suncoast hospice foundation fills the gap between reimbursable and non-reimbursable expenses associated with chronic or advanced illness. The foundation assists those who are not insured; those who need more than insurance covers; those who cannot afford to pay; those who have family members affected by a patients' health condition; and those who need assistance with needs outside or in addition to their direct medical diagnosis. Its volunteer board of trustees oversees efforts and provides leadership in the cultivation, solicitation and stewardship of its community stakeholders. In addition to providing financial resources to assist programs and services of empath health, the foundation engages in and with the community through support, volunteer engagement, and community involvement. Members of the staff and board are fully engaged in community activities through service to others, ensuring the organization knows and understands the needs of the communities in which it serves. Its social responsibility includes engagement activities such as the hospice beach stroll; aids walk; hospice gala; and other annual community-based and supported events.
