Form 990, Part VI, Section B, Line 11

The 990 is forwarded to each board member prior to filing with the irs. If the board member has any questions or concerns, they are instructed to contact the treasurer within seven days of receipt of the return. After the seven-day period for review, the tax preparer will finalize the return and send it to the treasurer. Upon final review it will be signed and filed with the irs.

Form 990, Part VI, Section B, Line 12C

Any director or officer must disclose a conflict of interest (1) prior to voting on any matter involving a conflict of interest, (2) prior to entering into any contract or transaction involving a conflict, (3) as soon as possible after the director or officer learns of the conflict, and (4) on the annual conflict of interest disclosure form. To enforce this policy, the secretary of the organization distributes annually to all directors and officers a form soliciting the disclosure of all conflicts of interest. The annual conlict of interest disclosure statements are reviewed each year to ensure that the organization is in compliance with its conflict of interest policy.

Form 990, Part VI, Section B, Line 15

At the completion of each fiscal year, top officials of the annika foundation prepare a detailed review of the prior year's programming results and financial performance which is submitted to all board members for review and comment. Compensation adjustments, if any, are determined by a consensus and resolution of the board.

Form 990, Part VI, Section C, Line 19
