| IRS990/Description | 0 | LIFE provides a Program of All-inclusive Care for the Elderly (PACE), an innovative model of long-term care that offers disabled and frail older adults the option of remaining at home with dignity and independence. The focal point of the program is the LIFE Center, where members socialize and receive many of the services offered by LIFE. The major services include: primary medical care and nursing care, all medications, rehabilitative therapies, nutrition services, therapeutic recreation, and much more. Additionally, LIFE offers in-home services including nursing, personal care, meal preparation, light housekeeping, home-delivered meals, and caregiver respite. LIFE also covers all medical specialists, laboratory and x-ray tests, and hospitalization when necessary. Transportation is provided to the LIFE Center and to medical appointments.LIFE opened in March 2010, with 7 members (enrollees), and added new members each month to accomplish a census of 66 members and 368 member months by year end. Each member received a comprehensive assessment by the interdisciplinary care team and an individualized plan of care and services. LIFE members are typically in their 70's or 80's and all are assessed as needing nursing home level care, but able to live safely in the community with LIFE services. LIFE succeeded in caring for these members in the community and reducing unnecessary hospital admissions and nursing home stays. As a PACE, LIFE is at risk, and pays for all medical and health services. The most expensive services include hospital and nursing home stays, medications, and home care. In 2011, LIFE will be able to provide more detailed utilization numbers. |
