IRS990/Description0LIFE provides a Program of All-inclusive Care for the Elderly (PACE), an innovative model of long-term care that offers disabled and frail older adults the option of remaining at home with dignity and independence. The focal point of the program is the LIFE Center, where members socialize and receive many of the services offered by LIFE. The major services include: primary medical care and nursing care, all medications, rehabilitative therapies, nutrition services, therapeutic recreation, and much more. Additionally, LIFE offers in-home services including nursing, personal care, meal preparation, light housekeeping, home-delivered meals, and caregiver respite. LIFE also covers all medical specialists, laboratory and x-ray tests, and hospitalization when necessary. Transportation is provided to the LIFE Center and to medical appointments.LIFE opened in March 2010, with 7 members (enrollees), and added new members each month to accomplish a census of 66 members and 368 member months by year end. Each member received a comprehensive assessment by the interdisciplinary care team and an individualized plan of care and services. LIFE members are typically in their 70's or 80's and all are assessed as needing nursing home level care, but able to live safely in the community with LIFE services. LIFE succeeded in caring for these members in the community and reducing unnecessary hospital admissions and nursing home stays. As a PACE, LIFE is at risk, and pays for all medical and health services. The most expensive services include hospital and nursing home stays, medications, and home care. In 2011, LIFE will be able to provide more detailed utilization numbers.

IRS990ScheduleO/GeneralExplanation/Explanation4Annually, all those serving the organization in a fiduciary capacity, including directors, officers, key employees and those having purchasing authority receive a copy of the policy and annual disclosure statement to be completed. Disclosures of financial interest or other reportable circumstances as defined in the policy are submitted and reviewed by the organization's Compliance Officer. The policy contains a continuing affirmative obligation on all affected individuals to disclose compensation or other circumstances throughout the year which may rise to the level of an actual or apparent conflict. The determination of whether a disclosed financial or other interest constitutes a conflict is made by the Compliance Officer. In consideration of a proposed transaction in which a conflict of interest has been identified, the CEO or Chairperson of the Board may, if deemed appropriate, appoint a dis-interested person or committee to investigate alternatives to the proposed transaction. Lastly, the policy addresses potential disciplinary action for violations of the policy. The policy is available to the public upon request.

The Medical Center Board/Committee has an established compensation philosophy which details the objectives of market positioning and pay elements. The committee engages with external consultants to provide market data comparing the organization's roles to similarly sized health systems utilizing both title and job content comparisons. The committee reviews the market analysis, approves any salary adjustments for the executive population, considers both reasonableness and effectiveness of all remunerative program and establishes the detailed performance expectations which are incorporated into the incentive plan. All of these discussions and decisions are documented through the provision of meeting minutes. CHE reviews and determines compensation for the Chief Executive Officer of the Medical Center.
