Founded in 2006, the patient-centered primary care foundation (the collaborative) is a not-for-profit membership organization dedicated to advancing an effective and efficient health system built on a strong foundation of primary care and the patient-centered medical home. The collaborative achieves its mission through the work of its fivestakeholder centers, led by experts and thought leaders dedicated to transforming the u.s. Health care system through delivery reform, payment reform, patient engagement, and benefit redesign. Today, the collaborative's membership has grown to over 1,200 diverse stakeholder organizations who represent health care providers across the care continuum, payers and purchasers, and patients and their families.

Mission

The patient-centered primary care foundation, d/b/a primary care collaborative (pcc), works to unify and engage its diverse stakeholders, including public and private organizations including physician and nursing groups, patient and consumer organizations, health plan, employers, industry, quality organizations, health systems and others, to promote polities and share best practices that support the growth of high-performing primary care and achieve the "quadruple aim": better care, better health lower costs, and greater joy for clinicians and staff in delivery of care.

While members do not have voting rights, pcc's executive members come together around key innovations, policies and programs to advance the vision of primary care embodied in the shared principles of primary care that is comprehensive, team-based and focused on patients and families.
