Urban Health Plan is network of federally qualified community health centers in the Bronx and Queens licensed under Article 28 of the New York State law. Its mission is to improve access to primary and specialty services for underserved communities.
Urban health plan is a federally qualified health center licensed under article 28 of the new york state health law with service locations in bronx, harlem and queens. In new york state, the organization has ten comprehensive service sites, eleven school health. Uhp provides a broad range of primary and specialty health care services to a largely medically underserved population. Uhp was founded in 1974 by dr. Richard izquierdo and over the past 40 years has undergone major quality, technological and operational improvements. The facilities are accredited by the joint commission and was recognized as a level 3 physician practice connections - patient centered medical home by ncqa. Uhp has been named one of the top 25 hispanic not-for-profits by hispanic business.
