Activity 2

Outreach and screening activities:the american kidney fund's screening and outreach programs, including akf's minority intervention and kidney education program (mike), partners with government agencies, community groups and faith-based organizations to provide free health education, kidney screenings and follow-up services &/or referrals for people who are at high risk for chronic kidney disease. Akf provided these services in at least 22 cities throughout the us in 2011. Akf's screenings are free of charge. Participants receive screenings to preliminarily test for hypertension, diabetes and kidney disease. All participants receive results and recommendations for appropriate action. High-risk participants-those found to have indicators of kidney damage-receive telephone contact, a follow-up survey, questions to ask their doctor and a resource list of medical care in the community. In 2011, akf screened 22,600 individuals. In 2011, akf partnered with 12 national organizations to mutually promote and educate the general public on kidney health.

Activity 3

Public education and prevention activities:the american kidney fund provides numerous services to increase public understanding of kidney health. The toll-free helpline (english/spanish) provides callers with information about kidney health, and financial assistance. In 2011 the helpline and our patient services department received nearly 43,000 contacts. The brochure, toolkit and fact sheet series includes 34 products addressing kidney health and related topics. More than 2,245,000 brochures and fact sheets were read, downloaded or distributed in 2011. One key piece of literature is the health education toolkit, which contains information which explains tests for kidney health, heart health, and diabetes and provides materials for community organizations, churches and others to deliver kidney disease prevention activities in their own communities. More than 3,625 toolkits were distributed in 2011.
