Applied design, research, and development has involved re- engineering hqp's longitudinal advanced preventive care model to provide an intensive 90-day post-acute home-based care model collaborating with patients, families, and cardiology specialists, including, as needed, accompanying patients to physician office visits. The model addresses the whole person; all of their medical conditions (in addition to heart failure) and psychosocial and environmental factors impacting their health.”
