Somerset community hospital, a tax-exempt affiliate of twin lakes center inc. Has a finance committee that is responsible for the oversight of the audit and selection of the independent auditors on behalf of twin lakes center inc. The process is consistent with prior years and has not changed.

FORM 990, PART III, LINE 4A:

Twin lakes center's main facility is located in somerset with outpatient locations in somerset, bedford, and cambria counties and a prevention location in somerset county. The facility provides treatment services throughout the state of pennsylvania. There are approximately 17 county contracts in place for public funding. The facility provides treatment regardless of the ability to pay. The primary funding source is from publicly funded sources. 2014/15 fiscal year summation: prevention staff was successful in offering services in all six federal strategies during the 2014/15 fiscal year. The percentage of recurring services was 67.06% which exceeded the 20% state mandated goal. The goal for state approved/evidence-based programs was also surpassed. The goal was set at 25% and twin lakes center ended the year at 37.5%. 2014/15 calendar year summation of activities - community service: - alcohol, tobacco and other drug (atod) educational classes were taught on a weekly basis to youth clients enrolled in two partial hospitalization programs at the children's aid home (estimated youth - 10). - evidence-based prevention curriculum, project towards no drug abuse, was taught to every, 8th grader in the meyersdale school district (estimated youth - 65). - evidence-based prevention curriculum botvin lifeskills (the top researched based prevention program in the country) was taught in seven county elementary schools, ten middle schools and one high school, reaching an estimated 2,286 youth. Educators from county school districts and staff from tlc were trained to teach botvin lifeskills at the middle school level (29). Funding sources for botvin lifeskills include united way of the laurel highlands $7,700, and university of colorado who provided an estimated $10,000 of support with a direct purchase of all level 1,2 & 3 student work books for the 2014/15 school year. Thirteen preschoolers received the evidence-based program healthy alternatives for little ones (halo), 22 students received alcohol true stories, and 52 adults received madd power of parents (a grant from the plcb ($4,206 year one 2014/15) was awarded to fund the expansion of this program. Also taught samhsa promising program - reach out now at one school (35). - supported national prevention campaigns including national drug fact week in january, inhalant awareness month and kick butts day in march, alcohol awareness month & prescription drug take back in april, national night out in august, fetal alcohol syndrome disorder day (fasd) in september, red ribbon week in october, great american smoke out (gaso) in november, and impaired driving prevention month in december. With each campaign, press coverage was provided and information was shared in face-to-face presentations and activities (reaching thousands of county residents). - taught drug and alcohol awareness and prevention classes weekly at the partial hospitalization program (gcc) at the bedford/somerset mhmr facility (estimated 50 clients yearly). - provide drug and alcohol prevention speaking engagements to community groups, schools, and human service agencies. Examples include: boys and girls club (50), school superintendents (12), tableland services (15), county commissioners board meeting (25), somerset borough council (27), sci staff empowerment days (100), somerset county rotary presentation (45), and administrative meetings at every school district in the county (36). - assisted in the administration of crn evaluations to dui offenders onsite at twin lakes center. - prevention staff serves as classroom instructors for alcohol highway safety school. This class is a requirement for all pa dui offenders (reached 165 county dui offenders). - provided technical support to all district school's students against destructive decisions (sadd) and teens against tobacco use (tatu) chapters. (provided scholarships to 52 sadd students to cover registration fee for the regional conference at seven springs - $2,340.00. Als

- in november, sadd advisor, jill rugg from sctc, was awarded the state's first sadd superior achievement award for the development and dedication of the county's only dui victim memorial site. - partnered with reality tour team for the start of this program in somerset county (estimated 120 of county residents experienced the tour. The reality tour won the harold w. Wheeler, jr. Memorial humanitarian award this year. A huge honor from the somerset county chamber of commerce). -- prevention department assembles and distributes family information bags at the conclusion of every reality tour (estimated 120). - supported wjac-tv mini-series on heroin epidemic. Conducted recorded interviews and participated in a live tv broadcast. - other steps taken to address national heroin epidemic in support of the ddap recommended community action plan: welcomed secretary tennis from ddap to somerset county for a meet and greet event (50 attendees); ran quarterly idu ads in the daily american; ran pastop.org ads in daily american and included in the reality tour information bags; hosted a naloxone training at the somerset borough building (estimated 70 attendees); supported heroin support groups and community seminars; also forwarded the published recommended prescribing guidelines of opiate pain medications to all county dentist offices, hospital emergency rooms and medical physician offices. - supported healthy alternative activity events at rockwood and conemaugh township high schools (reaching 400 youth). - welcomed to somerset county the edgar snyder save a life tour to five county schools (value of the cutting edge technology driven day-long presentations was in excess of $20,000). - worked in partnership with the somerset daily american to publish an 8-page newspaper supplement that included information on current drug trends, prevention initiatives, and a resource list outlining sources of help (reached 12,000 subscribers). Tobacco/nicotine initiatives: - offered smokeless saturday classes on a quarterly basis throughout the school year (39 attendees). - supported national kick butts day, great american smoke out, threw with chew, and world no tobacco day by hosting events and disseminating information. - after the great american smoke out presentation at dfc coalition meeting, sctc got immediate approval from their school board to have the language changed in their no use nicotine policy to include any nicotine delivery device or any device that mimics smoking. Press coverage was provided. - supported shanksville's "snuff out tobacco" day (125 students). - tobacco education was taught onsite for patients at twin lakes center once a week for all patients.
