Form 990, Part VI, Line 11B: Form 990 Review Process

A draft of the 990 was presented along with a draft of the financial statements for review by the board finance committee. The final 990 was available to all board members.

Form 990, Part VI, Line 12C: Explanation of Monitoring and Enforcement of Conflicts

CHC requires of staff and Board members yearly disclosure of outside relationships that might in anyway be considered to violate CHC Conflict of Interest policy. Employees have the responsibility to scrutinize actions in advance that might be considered in conflict, and to seek counsel from the Executive Director if there is any question (Employee Manual 6-3). Board members update their Conflict of Interest Policy forms at the September board meeting. Signed Conflict of Interest Policy forms are maintained in CHC's office.

Form 990, Part VI, Line 15A: Compensation Review & Approval Process - CEO, Top Management

CHC staff members are evaluated yearly by the Executive Director. Information is presented to the Board Personnel/Finance Committee for review and then is presented to the Board of Directors. The Executive Director is evaluated annually by the Board of Directors. Compensation levels are determined through consideration of performance, years of service, and position, in conjunction with a 2007 non-profit regional salary profile updated in 2011 in order to target a fair and reasonable salary as part of a search for the Executive Director position; additionally the Organization considers the salary profile guidelines published by PANO in 2011 in reviewing and determining a fair and reasonable Executive Director compensation level.

IN 2016-2017, MENTORING PROGRAMS WERE CONDUCTED IN 26 SCHOOLS AND REACHED OVER 978 STUDENTS. APPROXIMATELY 82% OF STUDENTS MAINTAINED/IMPROVED THEIR ABILITY TO RESIST/SAY NO TO PEER PRESSURE; 78% OF STUDENTS MAINTAINED/IMPROVED THEIR ABILITY TO COMMUNICATE WITH PEERS, UNDERSTAND ANOTHERS POINT OF VIEW, AND SPEAK CALMLY DURING A DISAGREEMENT; 77% MAINTAINED OR IMPROVED ABILITY TO SEEK HELP FROM ADULT; 75% MAINTAINED OR IMPROVED ABILITY TO HANDLE STRESS FROM HOME OR SCHOOL; 71% MAINTAINED OR IMPROVED BEHAVIOR; 66% MAINTAINED OR IMPROVED REPORT CARD GRADES; AND, 55% MAINTAINED OR IMPROVED ATTENDENCE.

THE PRIMARY GOAL OF SAP IS TO HELP STUDENTS OVERCOME BARRIERS TO SUCCESS IN ORDER THAT THEY MAY ACHIEVE, REMAIN IN SCHOOL, AND ADVANCE. LIASON SERVICES, TECHNICAL ASSISTANCE, MATERIAL DISSEMINATION, NEWSLETTERS, TEAM MAINTENANCE, AND EVALUATION ARE ACTIVE IN 52 SCHOOLS IN LEHIGH COUNTY. OUR COMMONWEALTH APPROVED TRAINER CERTIFIED 141 NEW SAP EDUCATION PROFESSIONALS WITHIN LEHIGH AND NORTHAMPTON COUNTIES.
