OFFERING INFORMATION AND ASSISTANCE, THE CENTERS ALSO REFER OLDER ADULTS TO OTHER RESOURCES THAT HELP ENSURE CONTINUED INDEPENDENT LIVING. TRANSPORTATION IS AVAILABLE FOR PEOPLE NEEDING A RIDE TO THE CENTER. WHILE THE CORONAVIRUS PANDEMIC REQUIRED SUSPENSION OF FACILITY-BASED ACTIVITIES INCLUDING IN-CENTER DINING IN MARCH, 2020, FISCAL YEAR 2020/2021, STILL SAW OVER 2,300 CONSUMERS PARTICIPATE IN ALTERNATIVE PROGRAMS VIA INTERNET AND SOCIAL MEDIA AS WELL AS OUTDOOR ACTIVITIES. ALSO, PANDEMIC-REQUIRED SERVICES INCLUDED "GRAB AND GO" MEALS (2,368) AND WELLNESS CALLS (3,768).FISCAL YEAR 2020 REPRESENTED THE SEVENTH YEAR OF AGING AT HOME, STARTED AS A DEMONSTRATION PROJECT IN PARTNERSHIP WITH UNITED WAY OF GREATER PHILADELPHIA AND SOUTHERN NEW JERSEY. ORIGINALLY, TARGETED TO THE "GREATER LANSDOWNE AREA", THE GREATER RIDLEY AREA WAS ADDED IN FISCAL YEAR 2018. THE INNOVATIVE "VILLAGE STYLE" PROGRAM TARGETED TO A LOW AND MODERATE INCOME POPULATION AIMS TO ENABLE OLDER PEOPLE TO REMAIN LIVIN GIN THEIR OWN HOME WITH INDEPENDENCE AND PEACE OF MIND. AS OF 2019/2020, OVER 900 "MEMBERS" WERE ENROLLED, RECEIVING DOOR-THRU-DOOR TRANSPORTATION, VOLUNTEER HANDYMAN SERVICES, VETTED HOME REPAIR REFERRALS AND VOLUNTEER HOME CHORE SERVICES. ALTHOUGH DEMONSTRATION FUNDING ENDED IN FISCAL YEAR 2018, AGING AT HOME IS NOW AN INTEGRAL COMPONENT OF SCS' SCOPE OF SERVICES.SCS SCOPE OF SERVICES ALSO INCLUDES:APPRISE HEALTH MEDICARE COUNSELING - APPRISE COUNSELORS PRESENTED INFORMATION ABOUT MEDICARE TO MORE THAN 7,500 DELAWARE COUNTY SENIORS AND FIELDED CALLS OR CONDUCTED ONE-ON-ONE COUNSELING AND BENEFITS COMPARISON SESSIONS WITH MORE THAN 1,700 SENIORS.COGNITIVE STIMULATION PROGRAM - SCS' INNOVATIVE BRAIN HEALTH KITS WERE THE FOCUS OF 750 ACTIVITIES IN LONG-TERM CARE FACILITIES.VOLUNTEER SERVICE - AS WITH MOST SCS SERVICES, THE VOLUNTEER PROGRAM REMAINED OPERATIONAL DURING 2020/2021, BUT WAS SCALED BACK TO MAINTAIN SAFETY DURING THE PANDEMIC. STILL, A TOTAL OF 8,827 VOLUNTEER HOURS WERE LOGGED DURING THE YEAR (OVER 40,000 HOURS IS TYPICAL). A BRIGHT SPOT IN MAY, 2021, THE HOLD ON THE VOLUNTEER HOMEBOUND MEAL DELIVERY SYSTEM WAS LIFTED AND, WITH MUCH ENTHUSIASM, SCS' DEDICATED VOLUNTEER DRIVERS AND "RUNNERS" RESUMED THEIR IMPORTANT ROLE DELIVERING VITAL NUTRITION TO FRAIL, ILL HOMEBOUND OLDER PERSONS.THE APPRISE HEALTH INSURANCE COUNSELING PROGRAM SUCCESSFULLY PIVOTED TO "TELEPHONE COUNSELING AND VIRTUAL GROUP PRESENTATION APPROACHES TO PERFORM ITS IMPORTANT SERVICE. DESPITE THE LIMITATIONS OF PANDEMIC CONDITIONS, ALMOST 1,500 OLDER PERSONS RECEIVED ONE-ON-ONE COUNSELING SESSIONS AND 127 GROUP OUTREACH/EDUCATION EVENTS WERE CONDUCTED WITH ALMOST 600 ATTENDEES. IT SHOULD BE NOTED THAT 2020/2021 SAW THE RESTRUCTURING OF THE STATE "TELE CENTER" PROGRAM TO A CENTRALIZED APPROACH. DELAWARE COUNTY HAD SERVED AS ONE OF NINE SITES IN THE COMMONWEALTH SINCE THE PROGRAM'S INCEPTION.COGNITIVE STIMULATION PROGRAMWITH LONG TERM CARE FACILITIES BEING THE PRIMARY SETTING FOR THE "KITS", THE PROGRAM WAS ESSENTIALLY DORMANT FOR MUCH OF 2020/2021. HOWEVER, THIS HIATUS PERIOD ENABLED A SUBSTANTIAL EFFORT OF KIT RENEWAL AND NEW DEVELOPMENT. THE NEW YEAR IS SEEING A RESUMPTION OF KIT DELIVERIES TO NURSING HOMES, ASSISTED LIVING FACILITIES AND SENIOR CENTERS IN DELAWARE COUNTY AND THE BROADER REGION. |
