Luke's health network, inc. Finance committee. Thereafter, the final federal form 990 was provided to each voting member of the organization's governing body prior to filing with the irs.”

In fiscal year 2023-2024 (FY 24), St. Luke's Penn Foundation served approximately 10,770 individuals and delivered 168,992 services. St. Luke's Penn Foundation also contracted with 85 businesses to provide their Employee Assistance Program (EAP). Approximately, 876 employees used the EAP service, and the team provided more than 3341.25 service hours of trainings, counseling, incident debriefings, and other consultative services. St. Luke's Penn Foundation manages the community petition site for crisis services for Bucks County, Pennsylvania. The petition site is located at St. Luke's Upper Bucks Campus (3000 St. Luke's Drive, Quakertown, PA 18951). “In the second year of a two-year, $3.9 million grant from the Substance Abuse and Mental Health Services Administration (SAMHSA) to implement the Certified Community Behavioral Health Clinic (CCBHC) model of care delivery, we made significant strides in expanding outreach, training staff, and connecting individuals to treatment. Highlights include the expansion of the Advisory Council, which is now 67% comprised of clients or family members of clients; outreach to 738 unique individuals to provide information about available programs and provide screenings around trauma and suicide risk; connection of 811 individuals to care with support from Navigation Services; and training of staff on topics including trauma, suicide prevention, care management for physical health, leadership, and culturally competent care for veterans and active-duty military personnel; and more. This year, we also received a $25,000 grant from Bucks County Drug & Alcohol Commission as part of its Opioid Settlement Funds to run a No Stigma campaign throughout Bucks County. The purpose of this campaign was to help connect individuals to treatment and reduce the stigma around addiction. St. Luke's Penn Foundation's Overview of Programs ------------------------------------------------- The broad continuum of care provided through the numerous programs identified below, along with community and health system partnerships, ensure comprehensive mental health care for the local community. The Mental Health Outpatient program provides comprehensive evaluations, counseling, and medication management for individuals, couples, and families presenting with mental health and co-occurring mental health and substance use disorders. Assertive Community Treatment (ACT) is a person-centered approach to care for adults 18 years of age and older who have a severe mental illness. By blending the disciplines of psychiatry, psychology, nursing, addiction, social work and psychosocial rehabilitation, the program offers a multidisciplinary team of professionals that works together to provide highly individualized treatment, outreach, rehabilitation, and support services in individuals' homes and communities. St. Luke's Penn Foundation operates three ACT teams: one in Sellersville (Bucks County), one in Pottstown (Montgomery County), and one in Exton (Chester County. Support is available 24 hours a day, 7 days a week. The ACT team in Sellersville serves residents of upper Bucks County and the Lansdale, Souderton, and Telford areas of Montgomery County. Admission requirements for Bucks County residents include involvement with the legal system or time spent in Bucks County prison. The Recovery Center offers a comprehensive continuum of inpatient and outpatient drug and alcohol services. The 55-bed onsite inpatient unit offers detoxification and rehabilitation in a structured environment. Outpatient options include general outpatient (individual, family, and group counseling), Intensive Outpatient Services (IOP), which involves individual and group counseling up to nine hours per week, and Mobile Engagement Services, a community-based addiction intervention service for individuals and families who fail to access or respond to traditional drug and alcohol treatment. The Warm Hand-Off program in Montgomery County and the BCARES (Bucks County Connect Assess Refer Engage Support) program in Bucks County operate in partnership with our county drug and alcohol agencies. The programs are designed to create support for overdose survivors presenting to the local emergency departments. A dedicated rapid access team works to connect individuals to treatment and to support their family members. St. Luke's Penn Foundation operates the Warm Hand-Off program at Abington Hospital - Jefferson Health and Abington-Lansdale Hospital and the BCARES program at Grand View Health, Doylestown Health, and St. Luke's Upper Bucks Campus. “Penn Villa is an integrated community providing housing support for adults with major mental illnesses and adults with co-occurring mental illnesses and substance abuse disorders. Within a caring community of current tenants, alumni, and committed staff, independence, personal growth, and holistic recovery are facilitated. Penn Villa serves eight individuals in its group home and 15 individuals within five townhomes. Penn Villa is an integrated community providing housing support for adults with major mental illnesses and adults with co-occurring mental illnesses and substance abuse disorders. Within a caring community of current tenants, alumni, and committed staff, independence, personal growth, and holistic recovery are facilitated. Penn Villa serves eight individuals in its group home and 15 individuals within five townhomes. The Village of Hope is a temporary housing program for homeless adults in Bucks County who have been diagnosed with both a mental health and substance abuse disorder. The goal of the program is to provide a supportive, recovery-oriented environment for those who voluntarily wish to recover from substance abuse and mental health issues. Residents learn skills necessary for successful re-entry into the mainstream of life. Two houses - one for men and one for women - each serve up to eight individuals and offer 24/7 staff support. Family Based Services provides 32 weeks of intensive community and home-based therapy and support for children and adolescents (up to age 21) with emotional and behavioral problems and their families. Children and adolescents in this program are typically at risk for out-of-home placement. Services focus on working with the family to successfully maintain the child in the home. Interventions include in-home therapy, casework services, family support services and 24/7 crisis management. Early Intervention services are designed to help families with children, from birth to age three, with developmental delays. Services, which include assessment, treatment planning, referral, and coordination of services, are individualized based upon the needs of each child and the child's family and are provided in the child's home, daycare center, or other settings familiar to the family. Staff members work with each family to provide them with strategies and ideas that they can easily incorporate into their child's existing routines. Pastoral Services provides direct chaplaincy services to all clients and residents requesting spiritual care. It is non-denominational in nature and is available to persons of all faiths. The Network targets a diversified asset allocation that places greater emphasis on equity-based investments to achieve its long-term objectives within prudent risk constraints. Endowment Spending Allocation and Relationship of Spending Policy to Investment Objectives The Board of Trustees of the Network determines the method to be used to appropriate endowment funds for expenditure. Calculations are performed for individual endowment funds at a rate of 4.5 percent of a three-year moving average market value with a minimum increase of 0% and a maximum increase of 10% per year over the previous year's spending amount. The total is reduced by the income distributed from the endowment fund in accordance with the preferences/restrictions made by the donors. The corresponding calculated spending allocations are distributed annually by June 30. In establishing this policy, the Board considered the expected long-term rate of return on its endowment. Accordingly, over the long term, the Network expects the current spending policy to allow its endowment to grow at an average of 8% percent annually, consistent with its intention to maintain the purchasing power of the endowment assets as well as to provide additional real growth through new gifts.”
