The board is provided with the form 990 for review before the return is officially filed.

Form 990, Part VI, Section B, Line 12C

The board reviews and signs the conflict of interest policy after the annual meeting, usually at or by the first meeting of the board. Those members of the board who are not present are mailed a conflict of interest package and are requested to return the signed forms at their earliest convenience. All new employees must sign the conflict of interest policy during orientation to avoid termination.

Form 990, Part VI, Section B, Line 15A

The president of the board meets with the executive director at least monthly to discuss any potential issues. The executive committee meets periodically during the year to discuss the executive director's performance. The president then meets with the executive director to discuss the outcome of the executive committee meeting and to offer an extension to the contract, if appropriate. Salary is based on a comparison with other same sized like agencies.

All governing documents are sent to the department of human services and the department of children & families along with the annual contract which is then made available to the public. The organization's financial statements are included in a combined audit report with the related organization disclosed in schedule r. There is a finance committee that assumes the responsibility of overseeing the audit and the financial statement preparation and the selection of the independent accountant. This process has not changed from prior year.

Financial Statement Notes

PART X, LINE 2:

U.s. Gaap requires management to evaluate tax positions taken by the organization and recognize a tax liability if the organization has taken an uncertain position that more likely than not would not be sustained upon examination by taxing authorities. Management evaluated the organization's tax positions and concluded that the organization had taken no uncertain tax positions that require adjustment to the financial statements to comply with the provisions of this guidance.
