Form 990, Part VI, Section B, Line 11B

The president, treasurer, and outsourced chief financial officer review the draft form 990 and request changes be made as necessary by the independent cpa firm who prepares the 990 tax return filing. Board members are provided with an annual copy of the conflicts of interest. Board members are required to disclose any conflicts of interest they are aware of on an annual basis and are provided with an annual copy of all organizational conflicts of interest.

Form 990, Part VI, Section B, Line 15A

Compensation of all staff members is reviewed as part of the annual budget and approved by executive committee and voted on by the full board. The compensation of the executive director is formally reviewed and approved by the executive committee and voted on by the full board.

Form 990, Part VI, Section C, Line 19
