Governance Explanations

FORM 990 PART VI, SECTION B, LINE 12C - Conflict of Interest Policy

Every new employee is required to sign a conflict of interest statement that will remain in effect throughout the employee's employment. Furthermore, each trustee and director must sign the conflict of interest statement every calendar year. The policy is reviewed and updated annually by Enable, Inc.

The Board of Trustees evaluates the Chief Executive Officer's performance annually and votes to approve the CEO's compensation at that time. Compensation is determined by a number of factors including performance and salary surveys conducted by various Human Resource Associations.

FORM 990, PAGE 2, PART III, 4A AND 4B- Program Services

Expanded PROGRAM descriptions: Enable currently delivers quality programs that are highly individualized to meet the needs of over 350 people with disabilities and the elderly throughout Central New Jersey. Personal choice is the foundation of all of Enable's services. 4A - Residential and Day services - Enable provides 24 hour support, 7 days a week, to 67 persons who live in group homes and supervised apartments. Residences are located in Mercer, Middlesex, Somerset, Monmouth, and Ocean Counties. A full array of personal care, help with being involved in social and recreational activities, and transportation is provided by staff. Day program services and supports were provided to 54 persons in two center based settings which focus on skill development and individualized and group educational activities or in community based settings which include more individualized activities in the community. In addition, a force of 1,025 volunteers completed 2,413 hours of volunteer supports helping to paint bedrooms and maintain yards, conducting one to one "friendly" visits, helping take someone to their place of worship, a local park, or shopping, and providing assistance at Enable's center based day programs. 4B - IN HOME SUPPORTS - Supports are tailored to individual needs. Services range from grooming and bathing to help with feeding, meal preparation and shopping, to assisting with transportation and going to appointments. Over 36,800 hours of personal care were provided to 238 persons. In addition, 3 home modification projects were completed to make bathrooms safer, add ramps or glides to stairs and improve the accessibility of the residence. In FY 15, Enable received funding from the NJ Division of Developmental Disabilities, the Department of Children and Families (DCF), and from Mercer County to provide respite care. These funds paid for 13,169 hours of respite relief for the caregiver which were provided by both Enable-hired respite workers and family-hired workers. Through a federal HUD ROSS grant, Enable provided case management supports to over 250 seniors and persons with disabilities living in 4 affordable housing developments in Princeton during FY15.

FORM 990, PART III - History and Philosophy

Enable, Inc. is devoted to helping men and women with disabilities (cognitive, physical and disabilities related to aging or disease) of all ages and seniors to remain in their own homes and live full and independent lives within our local communities throughout NJ. Founded in 1989 as a private, nonprofit affiliate, being the Mercer County Chapter of United Cerebral Palsy of New Jersey, the organization soon grew to serve individuals and families in a wider geographic area, presenting a wide range of disabilities. In 1994, the organization became Enable, Inc. PROGRAMS: Enable currently delivers quality programs that are highly individualized to meet the needs of over 350 persons with disabilities and the elderly throughout central New Jersey. Self-determination and personal choice are the foundation of all of Enable's services: RESIDENTIAL ASSISTANCE - Enable provides 24 hour support, 7 days a week, to 67 men and women who live in 15 group homes and 1 supervised apartment complex. Over 19,760 days of care were provided. Residences are located in Mercer, Middlesex, Somerset, Monmouth, and Ocean Counties. Oak Tree, a new group home, opened in the fall of 2014. A full array of personal care, supports for socialization and recreation, and transportation is provided by staff at all locations. The 2015 Consumer Satisfaction Survey results show 88% of residents and 82% of family members/caregivers were satisfied with the services they received. (NOTE: Overall, about 44% of persons served by Enable responded in the survey.) DAY PROGRAMS - Day services and supports were provided to 54 adults with disabilities for 7,112 days in center based setting or community based settings. This year Enable expanded Roszel Day Services, to accommodate 25 persons; opened a second Day Services in Flemington, NJ, which can accommodate people with developmental disabilities and special medical needs; and finalized plans for a third facility, Fusion in Middlesex Borough, which will accommodate 32 participants in FY2016. These weekday, daytime programs offer skill development, recreational, volunteer, social and educational opportunities for participants. The 2015 Consumer Satisfaction Survey results show that 100% of Day Services family members/caregivers and 94% of consumers were satisfied with the services they received. IN HOME SUPPORT - Supports are tailored to individual needs. They range from grooming and bathing to help with feeding, meal preparation and shopping, to assisting with transportation and conducting affairs in the community or providing respite for caregivers. 238 persons were served through over 36,800 hours of care. Of this group, 3families/individuals received house or apartment modifications that made bathrooms safer, stairs useable with ramps or glides added, and other accessibility improvements. Enable provides both agency-hired respite workers and fiscal intermediary support for families who choose to hire their own respite workers. Respite care is supported through funds from the NJ Division of Developmental Disabilities, the NJ Department of Children and Families and Mercer County. 94 families/individuals received 13,169 hours of respite relief for the caregiver. In FY2015, the State of New Jersey licensed Enable as a Health Care Services firm for seniors. Enable became DCF and Medicaid qualified to provide individual supports to children. In October 2015, Enable became DDD and Medicaid qualified to provide supports to adults with disabilities. In addition, a force of over 1,0259 volunteers (in FY2015) performed 2,413 hours of service to persons with disabilities helping to paint bedrooms and maintain yards, conducting one to one companionship visits, helping take someone to their place of worship, appointments or to recreation activities, and assisting at Enable's center based day programs. Form 990, Part III Organizational Structure and Funding

In 2011, Enable completed a strategic plan and set organizational goals that included preparing for changes in our industry. With the federal government's move towards "long term care" strategies for seniors and persons with disabilities and the State of New Jersey's Department of Human Services move from contract reimbursement into fee for service, Enable restructured. A leadership analysis was conducted resulting in redirecting more effort into quality assurance and business development. This year, three teams with members from diverse departments recommended tactics to prepare for the transition to Medicaid billing and Fee For Service. Teams identified the importance of training and cooperation in delivering quality services, excellent documentation, and comprehensive invoicing while meeting consumer service goals. Sharon Copeland, Chief Executive Officer, has served 40 years in social services and holds a Master's Degree in Social Work and is a licensed social worker. Stephen Rynn, Chief Operations Officer, oversees all program implementation efforts. Rynn previously held a senior position in an agency serving persons with disabilities in Staten Island for 20 years and holds a Master's in Social Work. Kathleen Patrick, who oversees compliance and development, has been in the field over 30 years and holds a Master's in Special Education. Lawrence Mitchell, Chief Financial Officer, has an MBA. Enable has implemented an electronic health record to track services and supports in preparation for a fee for service environment. Enable's employees consist of over 270 caring professionals. Services and supports are the heart of Enable's work; 86% of the budget is directed to program services; 13% supports administrative costs; 1% supports fundraising. This structure allows staff to manage and deliver services that are efficient, highly effective and based on the interests of consumers. Fundraising is critical to meet the various needs that arise that are not covered by state or private grants. Examples include providing additional dollars needed to purchase a wheelchair, adding handicapped accessible doors at one of Enable's programs, supporting the dynamic volunteer program, covering urgent repairs to homes when state funds are not available, and help to fund new program start up costs. Of its overall funding, 93% comes from government grants/contracts; 5% from private grants, contributions, and special events; and 2% from service fees. Enable's board of trustees representing community leaders, area corporations, small business, and legal and fiscal expertise oversees Enable's strategic direction, program accountability and fiduciary responsibilities. Additional community representatives serve on sub-committees that support the board's work.

FORM 990, PART XII, LINE 3A - Single audit and A-133 requirements

The organization does not receive federal awards of $500,000 or greater and is not required to complete a single audit in accordance with A-133. However, the organization does receive funding from the State of New Jersey and is required to complete a single audit in accordance with NJ 04-04-OMB which was done for the year ended June 30, 2015.

Schedule D Part X Line 2 - Income Tax Footnote

The Organization is exempt from Federal income taxes under Section 501(c)(3) of the Internal Revenue Code. Accordingly, no provision for income taxes has been recorded in the statement of activities and changes in net assets. The Organization had no unrecognized tax benefits at June 30, 2015 and 2014.
