PATIENTS ARE TAUGHT SYMPTOM AND MEDICATION MANAGEMENT AND ARE ASSIGNED HOME HEALTH CARE NURSES TO HELP WITH MEDICATIONS, DIET, DISEASE MANAGEMENT AND ASSISTING WITH LIFESTYLE CHANGES.(5) BREATHE EASY PROGRAM, ALLOWS PATIENTS WHO SUFFER FROM CHRONIC OBSTRUCTIVE PULMONARY DISEASE (COPD) TO BETTER MANAGE THEIR CONDITION AT HOME, IMPROVING THEIR OVERALL QUALITY OF LIFE.(6) TELEMONITORING, AN ADVANCED SYSTEM AVAILABLE TO REMOTELY MONITOR VITAL INFORMATION, SUCH AS BLOOD PRESSURE, HEART RATE, WEIGHT AND BLOOD OXYGEN LEVELS FOR CARDIAC PATIENTS OR ANY PATIENTS WHO NEED ADDITIONAL SUPERVISION.
