The Agency was founded in 1978 through a collaboration of interfaith community leaders committed to providing shelter to abandoned, abused, runaway and homeless youth. In recognition of the need, and the absence of available services for runaways, this group formed a volunteer organization with a Board of Directors and incorporated Crossroads as a private, non-profit Agency and opened Burlington County's first and only youth shelter. The Agency maintains its original mission and focus - empowering at-risk youth to develop into healthy young adults - as our core competency.Crossroads' Continuum of Care-----------------------------Crossroads' expertise and continuum of care for abandoned, abused, homeless and other at-risk youth - community based treatment homes, group homes, shelter care, life skills training, and mentoring - has resulted in improved outcomes and an enhanced future for the thousands of kids and families we have served over the years. Crossroads' residential and community - based services include.GROUP RESIDENTIAL HOME SERVICES - Total Expenses $ 3,572,350-------------------------------Programs-------- Rites of Passage (ROP) Cinnaminson House (CINN) Capable Adolescent Mothers (CAM) Dalton House (DH) Mount Holly House (MHH) Kerri's House(KH) For Youth Aging-Out of the Child Welfare SystemRites of Passage Transitional Housing Program (ROP) --------------------------------------------------- Nineteen(19)homeless youth, ages 17 to 21, received independent living services including supportive housing, intensive life skills training, job coaching, and cash management services. During the year 3,496 bed day units of group home service were provided to program youths, with 90.6 % achieving their employment, educational and life skills goals.Cinnaminson House (CINN) ------------------------ Focusing on school and work achievements 26 youth, ages 16 to 21 years old, received an opportunity to hone the skills they need for independent living as they transition to adulthood. During the year 4,074 bed day units of group home service were provided to program youth, with 92.2 % achieving their employment, educational and life skills goals. For Emotionally-Behaviorally Disordered YouthCapable Adolescent Mothers (CAM)-------------------------------- Ten (10) pregnant mothering teen girls, ages 16 to 21 and 8 infants, stabilized their emotional and behavioral health by participating in individual and group therapy, attending school, and receiving life skills and parenting training. During the year 2,265 bed day units of group home and treatment home services were provided to program teenage moms, with 94.5 % achieving their life-skills, emotional, educational, behavioral and parenting goals and 1,943 units of service were provided to babies. Dalton House (DH) --------------------------------- Individual and group therapy, case management, employment coaching and life skills development services helped 22 teens, ages 16 to 21, stabilize their emotional and behavioral health. During the year 3,301 units of group home service were provided to program youth with 78.5 % achieving their life skills, emotional, educational and behavioral goals.Mount Holly House (MHH)-----------------------Fourteen (14) teen girls in our residential treatment center, ages 14 to 17, stabilized their behaviors and emotional reactions in order to help them acquire basic life skills, achieve some academic success, return home or transition to independent living and achieve self-sufficiency. During the year 1,792 bed day units of RTC services were provided to program girls with 89% of their goals achieved.

During the year 3,471 units of treatment home services were provided to program youth, with 82.4 % achieving their educational, emotional and behavioral goals.Project DISCOVERY----------------- Specialized treatment services were provided to 5 emotionally disturbed youth who also self-identify as LGBTQ (lesbian, gay, bisexual, trans gender or questioning). The treatment homes provided a therapeutic family environment to help ameliorate emotional, social and/or youth behavioral difficulties and facilitate their stabilization in the community. During the year 1,101 units of treatment home services were provided to program youth, with 83.7 % achieving their educational, emotional and behavioral goals. Flexible Solutions (FLEX)------------------------- Specialized foster care services were provided to 55 youth, offering them safe, stable, temporary placement during an emergency situation or for a respite period. During the year 6,625 bed days units of treatment home services were provided to program youth, with 80.3 % achieving their educational, emotional and behavioral goals.Atlantic County Flex (AC)----------------------- 472 bed days of specialized foster care treatment home services were provided to Fifteen (15) Atlantic County youth that were in need of temporary out of home placements with 89.3 % achieving their educational, emotional and behavioral goals. For Teen Parents ----------------Second Chance Homes (SCH)-------------------------- A homeless teen mother and her children were given the opportunity for normal family and community life while they worked toward independent living. The program delivered extensive care management services,and allowed these teens to attend school, obtain child care, develop job and life skills and learn parenting, with 89 % success in achieving their life skills, educational and parenting skills.

During the year, 1,368 hours of intensive mentoring or group services were provided to program youth, with 73.5 % achieving their social, emotional and behavioral goals.Positive Choices (PC) - Burlington County----------------------------------------- Our PC program for 11-13 year olds provided 820 community education service hours to 74 youth during the year ended 8/31/17 with 74 % achieving their life skills, emotional and behavioral goals. Our PC program for 14-17 year olds provided 603 community service hours to 50 youth during the year ended 8/31/17 with 97.7 % achieving their life skills,emotional and behavioral goals.Camden County - The Young Men of Honor--------------------------------------Beginning April 2017 to August 31, 2017, the program provided 167 hours of positive youth development prevention services to twelve (12) Camden County males ages 9 to 18.
