Crossroads empowers youth who are homeless, abandoned, abused or at-risk, to lead healthy, productive lives.

Major Activities

Activity 2
THERAPEUTIC FOSTER CARE (TREATMENT HOME) SERVICES------------------------------------------------- For Homeless and Runaway Youth ------------------------------ Trinity Place Host Homes (TPHH)------------------------------- A safe haven in a family home was provided for six runaway and homeless youth while family reunification was negotiated or a safe placement was located and other needed services were arranged. During the year, 46 units of shelter services were provided to program youth, with 100 % moving on to safe, stable housing. In addition, 324 and 196 program service hours were provided for prevention family/youth counseling and community education, respectively. For Emotionally-Behaviorally Disordered Youth ---------------------------------------------Community Care for Kids (CCK)----------------------------- Twenty-three emotionally disturbed children who were at risk of being placed in a residential treatment facility were given the opportunity for normal family and community life while receiving therapeutic services. During the year 4,093 units of treatment home services were provided to program youth, with 83 % achieving their educational, emotional and behavioral goals.Project DISCOVERY----------------- Specialized treatment services were provided to four emotionally disturbed youth who also self-identify as LGBTQ (lesbian, gay, bisexual, transgender or questioning). The treatment homes provided a therapeutic family environment to help ameliorate emotional, social and/or youth behavioral difficulties and facilitate their stabilization in the community. During the year 1,014 units of treatment home services were provided to program youth, with 83 % achieving their educational, emotional and behavioral goals. Flexible Solutions (FLEX)------------------------- Specialized foster care services were provided to 18 youth, offering them safe, stable, temporary placement during an emergency situation or for a respite period. During the year 1,059 units of treatment home services were provided to program youth, with 77 % achieving their educational, emotional and behavioral goals.Detention Alternative Bed Days ------------------------------ This program provides temporary shelter, supevision, crisis intervention and cash management to youth who would otherwise be released but do not have suitable parent/guardian supervision at the time of their detention admission intake call while awaiting a court appearance. One community youth was provided 1 bed day unit of service during the year. For Teen Parents ----------------Second Chance Homes (SCH)-------------------------- Three homeless teen mothers and their children were given the opportunity for normal family and community life while they worked toward independent living. The program delivered 70 units of extensive care management services, allowed these teens to attend school, obtain child care, develop job and life skills and learn parenting, with 100 % achieving their life skills, educational and parenting skills.

COMMUNITY BASED SERVICES------------------------ For Youth Aging-Out of the Child Welfare System -----------------------------------------------Just for You Learning Lab (JFY)------------------------------- The program helped 42 youth in foster care, ages 14 to 17, prepare for independent living by exposing them to the basic life skills and tools they'll need to function independently someday. Services included exposure to employment and volunteer opportunities, peer networking, and cultural events. During the year, 6,366 units of service were provided to program youth, with 87% demonstrating progress in their life development goals. For At-Risk Families & Children -------------------------------Preteen Promise (PTP)--------------------- The program provided intensive mentoring services to 9 at-risk preteen girls, ages 11 to 13, and their families, improving their interpersonal, emotional and social skills as demonstrated by a reduction of behavioral problems at school and, ultimately, preventing their involvement in the juvenile justice system. During the year, 1,539 hours of intensive mentoring or group services were provided to program youth, with 95 % achieving their social, emotional and behavioral goals.Nurturing Parents----------------- This program is a mentor program that offers supportive coaching and counseling for the parents of at-risk preteen girls (from Preteen Promise) with the goal of increasing positive behaviors in their daughters by incorporating family involvement. Eight parents were provided 1,902 hours of services with 91 % achieving their social, emotional an behavioral goals during this program start-up year.

IRS990/Description0Crossroads Programs, Inc.(www.crossroadsprograms.org) has been a provider of residential, shelter, transitional, and supportive services for homeless abandoned, abused and at risk youth and families for over 35 years. The Agency was founded in 1978 through a collaboration of interfaith community leaders committed to providing shelter to abandoned, abused, runaway and homeless youth. In recognition of the need, and the absence of available services for runaways, this group formed a volunteer organization with a Board of Directors and incorporated Crossroads as a private, non-profit Agency and opened Burlington County's first and only youth shelter. The Agency maintains its original mission and focus - empowering at-risk youth to develop into healthy young adults - as our core competency.Crossroads' Accreditation -------------------------Crossroads has been accredited by the Council on Accreditation for Child & Family Services (COA) since 2000. In mid 2012 the Agency received expedited reaccreditation due to our compliance with all standards in the three-day, three-person review. Crossroads was recognized by COA as "provider of services of high quality" and in all organizational functions (governance, quality improvement, finance, human resources, staff development, rights & ethics, and risk prevention) and for seven (7) different types of programming:* Foster Care Services * Group Living Services* Youth Independent Living Services * Counseling, Support and Educ. Services * Child and Family Development and Support Services* Shelter Services * Supported Community Living ServicesCrossroads is Proudly Accredited through June 30,2016Crossroads' Continuum of Care-----------------------------Crossroads' expertise and continuum of care for abandoned, abused, homeless and other at-risk youth - community based treatment homes, group homes, shelter care, life skills training, and mentoring - has resulted in improved outcomes and an enhanced future for the hundreds of kids and families we have served over the years. Crossroads' residential and community - based services include.GROUP RESIDENTIAL HOME SERVICES--------------------- For Youth Aging-Out of the Child Welfare SystemRites of Passage Transitional Housing Program (ROP) --------------------------------------------------- Fifteen homeless youth, ages 17 to 21, received independent living services including supportive housing, intensive life skills training, job coaching, and cash management services. During the year 3,537 units of group home service were provided to program youths, with 91% achieving their employment, educational and life skills goals.Cinnaminson House (CINN) ------------------------ Focusing on school and work achievements 27 youth, ages 16 to 21 years old, received an opportunity to hone the skills they need for independent living as they transition to adulthood. During the year 3,728 units of group home service were provided to program youth, with 84 % achieving their employment, educational and life skills goals. For Emotionally-Behaviorally Disordered YouthCapable Adolescent Mothers (CAM)-------------------------------- Twenty two pregnant mothering teen girls, ages 16 to 21 and 19 infants, stabilized their emotional and behavioral health by participating in individual and group therapy, attending school, and receiving life skills and parenting training. During the year 2,868 units of group home service were provided to program youth, with 93 % achieving their lifeskills, emotional, educational, behavioral and parenting goals and 2,347 units of service to babies. Dalton House (DH) --------------------------------- Individual and group therapy, case management, employment coaching and life skills development services helped 23 teens, ages 16 to 21, stabilize their emotional and behavioral health. During the year 3,480 units of group home service were provided to program youth with 91 % achieving their life skills, emotional, educational and behavorial goals.
