THE COASTAL PLAIN RURAL HEALTH NETWORK (CPRHN) IS A VERTICAL NETWORK THAT SERVES CLARENDON, FLORENCE, MARION, SUMTER AND WILLIAMSBURG COUNTIES THROUGH PROJECTS AND PROGRAMS THAT INCREASE HEALTH AWARENESS AND ACCESS TO MEDICAL CARE. WITH PAST SUPPORT FROM THE DUKE ENDOWMENT, PASOS THE NETWORK ENSURES THAT PATIENT POPULATIONS GET THE NECESSARY EDUCATION, SCREENINGS AND NAVIGATION THROUGH THE COMPLICATED HEALTHCARE SETTING. THE CPRHN WAS ORGANIZED WITH THE FINANCIAL AND TECHNICAL ASSISTANCE OF THE SOUTH CAROLINA OFFICE OF RURAL HEALTH (SCORH). THIS NETWORK IS ONE OF SIX NETWORKS DEVELOPED THROUGHOUT SOUTH CAROLINA AND SCORH. SCORH IS A 501 (C) (3) ORGANIZATION DEDICATED TO IMPROVING ACCESS TO QUALITY HEALTHCARE IN RURAL COMMUNITIES ACROSS SOUTH CAROLINA. THE CPRHN PARTICIPATES IN A VARIETY OF HEALTH RELATED EVENTS THROUGHOUT THE YEAR SUCH AS; HEALTH FAIRS, HEALTH SCREENINGS, EDUCATION FORUMS, FOCUS GROUPS AND COMMUNITY OUTREACH.DURING 2017 THE CPRHN OFFERED THE FOLLOWING PROGRAM SERVICES TO NETWORK MEMBERS:PASOSPASOS IS A COMMUNITY BASED ORGANIZATION FOUNDED IN 2005 AND DEVELOPED TO HELP LATINO COMMUNITY MEMBERS AND SERVICE PROVIDERS WORK TOGETHER FOR STRONG AND HEALTHY LATINO FAMILIES. PASOS PROVIDES APPROPRIATE EDUCATION ON FAMILY HEALTH AND POSITIVE PARENTING SKILLS; INDIVIDUAL GUIDANCE FOR PARTICIPANTS IN NEED OF RESOURCES; AND PARTNERSHIP WITH HEALTH CARE AND SOCIAL SERVICES PROVIDERS TO HELP THEM PROVIDE MORE EFFECTIVE SERVICES TO THIS POPULATION. PASOS CULTIVATES THE SKILLS OF GRASSROOTS COMMUNITY LEADERS. TEAMS OF PROMOTORES, OR COMMUNITY HEALTH WORKERS, ARE BEING DEVELOPED ACROSS THE STATE AND OUR REGION TO HELP EDUCATE THEIR COMMUNITIES AND CONNECT THEM TO HEALTH RESOURCES. THE CPRHN HAS BEEN AFFILIATED WITH PASO'S SINCE 2010 WITH PROGRAMS LOCATED IN CLARENDON AND WILLIAMSBURG COUNTIES.HEALTHY FAMILIESTHE HEALTHY FAMILIES PROGRAM IMPROVES THE MATERIAL AND CHILD HEALTH OF LIMITED ENGLISH PROFICIENCY (LEP) LATINO FAMILIES THROUGH CULTURALLY APPROPRIATE METHODS, WHICH INCLUDE PATIENT NAVIGATION SERVICES TO OVER 400 WOMEN AND CHILDREN. THE CPRHN PARTNERED WITH CLARENDON HEALTH SYSTEM TO PROVIDE NAVIGATING SERVICES ALONG WITH INCREASED ACCESS TO CARE FOR THE MOTHER AND BABY, IMPROVED QUALITY AND ACCESS OF THE DELIVERY PROCESS, IMPROVED COMMUNICATION BETWEEN PATIENT, PROVIDER AND AT THE DELIVERING HOSPITAL. THESE PROGRAMS HAVE STRENGTHENED THE INFRASTRUCTURE FOR HEALTH CARE DELIVERY TO LEP, SPANISH SPEAKING HISPANIC WOMEN ACROSS THE FIVE COUNTY REGION. THE CPRHN HAS BEEN AFFILIATED WITH PASO'S SINCE 2010 WITH PROGRAMS LOCATED IN CLARENDON AND WILLIAMSBURG COUNTIES.HEALTHY FAMILIESTHE HEALTHY FAMILIES PROGRAM IMPROVES THE MATERIAL AND CHILD HEALTH OF LIMITED ENGLISH PROFICIENCY (LEP) LATINO FAMILIES THROUGH CULTURALLY APPROPRIATE METHODS, WHICH INCLUDE PATIENT NAVIGATION SERVICES TO OVER 400 WOMEN AND CHILDREN. THE CPRHN PARTNERED WITH CLARENDON HEALTH SYSTEM TO PROVIDE NAVIGATING SERVICES ALONG WITH INCREASED ACCESS TO CARE FOR THE MOTHER AND BABY, IMPROVED QUALITY AND ACCESS OF THE DELIVERY PROCESS, IMPROVED COMMUNICATION BETWEEN PATIENT, PROVIDER AND AT THE DELIVERING HOSPITAL. THESE PROGRAMS HAVE STRENGTHENED THE INFRASTRUCTURE FOR HEALTH CARE DELIVERY TO LEP, SPANISH SPEAKING HISPANIC WOMEN ACROSS THE FIVE COUNTY REGION. THE DUKE ENDOWMENT FUNDED THIS PROGRAM THRU 2017.
