Mission and Program Overview

Mission

Gateway to hope is a breast cancer lifeline. Our mission is to secure comprehensive care and provide financial assistance for individuals in need diagnosed with or at high risk for developing breast cancer. (see schedule o)almost everyone is touched by breast cancer in one way or another. Breast cancer sufferers often encounter a decrease or loss of income due to their illness. Gateway to hope helps individuals diagnosed with breast cancer who fall at or below 450% of the federal poverty guidelines, and receive treatment in missouri. Gateway to hope pays for their insurance premiums from diagnosis to treatment completion as well as providing financial assistance for lymphedema compression garments, and living expenses such as rent, transportation, utilities, and prescription drugs. A team of registered nurses and master social workers manage each case with consistent communication from diagnosis to treatment completion via phone or in person.

Provide no cost comprehensive breast cancer care for uninsured/underinsured individuals.

Governance Explanations

Form 990, Part VI, Section A, Line 2

David caplin, director/founder, and john caplin, director have a family relationship.

Form 990, Part VI, Section A, Line 4

The organization adopted a resolution to create an executive committee and a healthcare landscape committee. The executive committee is comprised of 4 persons, including president, vice president, treasurer, and secretary. The healthcare landscape committee will have a chair and 4 directors nominated by president and approved by board of directors.

Form 990, Part VI, Section B, Line 11

Prior to filing the 990 with irs a draft copy of the return is provided to the executive director for review with the president and treasurer of the board. Changes, if any, are communicated and incorporated into the 990. A final copy is then provided to the president of the board of directors to be approved for filing.

Form 990, Part VI, Section B, Line 12C

Gateway to hope regularly and consistently monitors and enforces compliance with the conflict of interest policy by explaining the policy to board members at meetings and by having each member sign a statement that they will disclose any conflict of interest.

Form 990, Part VI, Section B, Line 15

The process for determining compensation of the executive director and other key employees includes use of a compensation committee, form 990 of other organizations, and compensation surveys. The board of directors or compensation committee must approve the compensation. At this time the organization does not compensate any reportable key employees.

Form 990, Part VI, Section C, Line 19

Upon request, the organization makes its governing documents, conflict of interest policy and financial statements available to the public.
