Effective june 3, 2021, college success arizona was renamed education forward arizona. The articles of incorporation were amended to reflect this change.

The form 990 is prepared by an outside accounting firm and then reviewed and approved by the president & ceo and board of directors before it is filed with the irs.

Form 990, Part VI, Section B, Line 12C

The board of directors are required to complete and sign two conflict of interest questionnaires annually. The ceo and treasurer review and approve all transactions of the organization according to the organization's conflict of interest policy. Any potential conflicts of interest would be detected during the review and approval process. The procedures stated in the conflict of interest policy would be followed to deal with any issue arising.

The current salary of the president & ceo and executive vp & coo was determined by the experience of each individual and a review of the current council on foundations salary and benefit study. The board of directors make the determination each year if salary increases are warranted. Other salaries are reviewed by the ceo & coo at the same time.

FORM 990, PART III, PROGRAM SERVICE ACCOMPLISHMENTS:

On february 1, 2021, college success arizona (csa) assumed the financial assets and the staff for expect more arizona and achieve60az and those two entities would now be under the umbrella of csa. This transfer of assets would allow for the organization to create a new name, logo, and a strategic plan that would focus on commitment to diversity, equity and inclusion so that marginalized communities are being provided educational opportunities to maximize their success aligned with an education agenda leading to a stronger collective voice for educational improvement and shared accountability for all arizonans. On august 5, 2021, we launched the new name of the organization as education forward arizona. This new organization is a reflection of what business, education and community leaders across the state are saying: if we want to move our state forward, we must move education forward. Education forward arizona seeks to change the way people think about and support education as the key to improving the economy and the quality of life in arizona. We know that arizonans want bold action that moves education forward in arizona. Together, we will advocate for and act on education improvements that advance the quality of life for all arizonans by: -creating a new story about education in arizona so that policymakers, businesses and community leaders see education as an investment in a better future and not an expense. -scaling innovative programs, such as ask benji, adviseaz, college knowing and going, and publishing data to drive decision-making. -partnering with change agents and courageous champions across the state who will advocate for actions that will close achievement gaps. Our high-level organizational goals include: -develop support for improved outcomes on arizona education progress meter indicators -reach the state attainment goal by 2030, working through the p-20 continuum -close achievement gaps at all levels -advocate for state-level funding allocated to those most in need education is the fundamental driver of a more robust arizona workforce and economy, and our best lever for improving the quality of life for all arizonans. To move arizona forward, we must move education forward.

Financial Statement Notes

PART V, LINE 4:

The endowment funds are held as investments. The earnings from the investments are held for scholarship grants and college success service expenses. These expenses include professional and peer mentoring, as well as technical and program costs.

PART X, LINE 2:

The organization qualifies as a tax-exempt organization under section 501(c)(3) of the internal revenue code ("irc") and, accordingly, there is no provision for income taxes. In addition, the organization qualifies for the charitable contribution deduction under section 170 of the irc and has been classified as an organization that is not a private organization. Income determined to be unrelated business taxable income would be taxable. The organization evaluates its uncertain tax positions, if any, on a continual basis through review of its policies and procedures, review of its regular tax filings, and discussions with outside experts. As of december 31, 2021 and 2020, management does not believe any uncertain tax positions exist. The organization's federal return of organizations exempt from income tax (form 990) for 2018, 2019, and 2020 are subject to examination by the irs, generally for the three years after they were filed.

SUCCESS ADVISERS MEET WITH EACH SCHOLAR AT LEAST TWICE PER SEMESTER IN-PERSON ON THEIR RESPECTIVE CAMPUS AS WELL AS BY PHONE AND VIRTUALLY, HELPING THEM TO IDENTIFY OBSTACLES TO COMPLETING THEIR COLLEGE EDUCATION AND ASSISTING THEM IN CREATING A PLAN TO OVERCOME THOSE OBSTACLES. THESE BARRIERS OR OBSTACLES COULD BE FINANCIAL, FAMILY SUPPORT, STUDY HABITS, HOUSING, EMOTIONAL AND MENTAL HEALTH ISSUES, ETC. (CONTINUED ON SCHEDULE O)SUCCESS ADVISERS PROVIDE SUPPORT AND ENCOURAGEMENT TO THEIR SCHOLARS, OFTEN REMINDING THEM THAT THEY WERE CHOSEN FOR THIS SCHOLARSHIP BECAUSE EDUCATION FORWARD ARIZONA BELIEVES IN THEM AND THEIR FUTURE. SINCE ITS INCEPTION IN 2005, THE SUCCESS SERVICES PROGRAM HAS PRODUCED OVER 800 GRADUATES. WE CURRENTLY HAVE A CUMULATIVE 6-YEAR GRADUATION RATE OF 74% AND A FRESHMAN RETENTION RATE OF 91%. APPROXIMATELY 70% OF OUR STUDENTS ARE FIRST GENERATION COLLEGE STUDENTS. THESE OUTCOMES COMPARE FAVORABLY WITH ALL STATE UNIVERSITIES IN ARIZONA. THE SUCCESS SERVICES PROGRAM ALSO HOSTS AN ANNUAL LEADERSHIP SYMPOSIUM FOR OUR SCHOLARS EACH JANUARY. THIS ALL-DAY EVENT IS AN OPPORTUNITY FOR SCHOLARS TO MEET AND NETWORK WITH FELLOW STUDENTS FROM OTHER COLLEGE CAMPUSES, HEAR FROM INSPIRATIONAL SPEAKERS, PARTICIPATE IN TEAMBUILDING ACTIVITIES, AND ATTEND VARIOUS WORKSHOPS FOCUSED ON PREPARING FOR LIFE AFTER COLLEGE. THE COLLEGE ACCESS PROFESSIONAL COMPONENT OF AZCAN (ARIZONA COLLEGE ACCESS NETWORK), CURRENTLY HAS 958 MEMBERS. DURING 2021, WE DELIVERED 17 PROFESSIONAL DEVELOPMENT MODULES TO 415 PROFESSIONALS WITH 100 EARNING CERTIFICATES AS ARIZONA COLLEGE ACCESS PROFESSIONALS. 174 OF OUR AZCAN MEMBERS REPRESENT COLLEGE ACCESS PROGRAMS, 27 REPRESENT COMMUNITY COLLEGES, 24 REPRESENT UNIVERSITIES, 313 REPRESENT K-12, 61 REPRESENT SCHOOL DISTRICTS, 6 REPRESENT STATE AGENCIES, 3 REPRESENT TECHNICAL/TRADE SCHOOLS, AND 350 REPRESENT VARIOUS NONPROFITS, BUSINESSES, AND CORPORATIONS. AZCAN EXPANDED THE DEVELOPMENT OF THE NEW COMMUNITY-BASED COLLEGE READINESS TEAMS IN 2021 AS PART OF THE HELIOS EDUCATION FOUNDATION'S COLLEGE KNOWING & GOING INITIATIVE (CKG). THESE TEAMS - MADE UP OF HIGH SCHOOL ADMINISTRATION, COUNSELORS, TEACHERS, AMERICORPS MEMBERS, POSTSECONDARY PARTNERS, COMMUNITY MEMBERS AND CKG COORDINATORS - COLLABORATE TO IDENTIFY EFFECTIVE STRATEGIES FOR INCREASING COLLEGE READINESS AND ACCESS AND STRENGTHENING THE COLLEGE-GOING CULTURE IN WAYS THAT ARE SPECIFIC TO THE NEEDS OF THEIR COMMUNITIES.AZCAN LAUNCHED YEAR 3 OF THE ADVISEAZ AMERICORPS PROGRAM TO SUPPORT 50 ADVISEAZ MEMBERS IN THEIR WORK TO HELP MORE THAN 2,500 STUDENTS WHO ARE LOW-INCOME, FIRST-GENERATION AND/OR STUDENTS OF COLOR.
