Horizon village's five (5) treatment centers, comprised of 193 total beds, provides intensive chemical dependency services to residents of all eight (8) counties of western new york. It is designated for chemically dependent adults 18 and older (50 beds), chemically dependent us veterans (25 beds), and young adults (age 18-25) (25 beds) who have been unable to establish abstinence through outpatient or traditionally inpatient treatments. Terrace house is a 68 bed facility dedicated to medically supervised detoxification (10 beds), crisis stabilization (58 beds) for chemically dependent adults.

The 990 is presented in draft form to the members of the governing body of horizon village, inc. After it has been reviewed by agency fiscal personnel. Each member of the board has the opportunity to review the return and suggest any changes prior to filing with the internal revenue service.

Form 990, Part VI, Section B, Line 12C

In accordance with horizon village inc.'s conflict of interest policy, each "officer" includes any non-director serving on any committee of the board of directors and directors shall annually sign a statement which affirms that such person: a. Has received a copy of the conflict of interest policy, b. Has read and understands the policy, c. Has agreed to comply with the policy, and d. Understands the corporation is a charitable organization and that in order to maintain its federal tax exemption, it must engage primarily in activities which accomplish one or more of its tax-exempt purposes, e. Is not aware that he or she has any financial interest or, if he or she is aware of any financial interest, a description of all material fact concerning it. The organization updated its conflict of interest policy (coi) in 2014 to conform with the requirements of the nys non-profit revitalization act of 2013. In accordance with the procedures stated in the coi policy, board members and officers: 1. Obtained a copy of the policy; 2. Reviewed potential conflicts (as defined) 3. Disclosed potential conflicts or confirmed no conflict exists 4. Recused themselves from discussion and voting (if conflict exists) the board of directors monitored and enforced compliance with the coi policy and documented this in minutes to meetings.

The compensation committee is charged by the board of directors with the responsibility to determine the compensation of the chief executive officer, and approval of the amounts and types of compensation of the senior leadership team. The purpose of this committee is to: approve the annual performance standards of the chief executive officer. Determine appropriate performance measures and establish criteria for any incentive based compensation. Conduct the annual performance appraisal for the chief executive officer. Determine the total compensation package for the chief executive officer. Annually review, and approve any change to the total compensation of the chief executive officer and senior leadership team.

The companies are exempt from taxation under section 501 (c)(3) of the internal revenue code. It is highly certain that some positions taken for income tax purposes would be sustained upon examination by the taxing authorities, while others are subject to uncertainty about the merits of the position taken or the amount of the position that would be ultimately sustained. The companies are not aware of any uncertain tax positions as of december 31, 2022 or 2021. HORIZON VILLAGE'S FIVE (5) TREATMENT CENTERS, COMPRISED OF 193 TOTAL BEDS, PROVIDES INTENSIVE CHEMICAL DEPENDENCY SERVICES TO RESIDENTS OF ALL EIGHT (8) COUNTIES OF WESTERN NEW YORK. IT IS DESIGNATED FOR CHEMICALLY DEPENDENT ADULTS 18 AND OLDER (50 BEDS), CHEMICALLY DEPENDENT US VETERANS (25 BEDS), AND YOUNG ADULTS (AGE 18-25) (25 BEDS) WHO HAVE BEEN UNABLE TO ESTABLISH ABSTINENCE THROUGH OUTPATIENT OR TRADITIONALLY INPATIENT TREATMENTS. TERRACE HOUSE IS A 68 BED FACILITY DEDICATED TO MEDICALLY SUPERVISED DETOXIFICATION (10 BEDS), CRISIS STABILIZATION (58 BEDS) FOR CHEMICALLY DEPENDENT ADULTS.
